노인보건제도와 노인간병 요양원 제도 개발의 과제와 전망

국문 초록

우리나라의 경우 인구 노령화가 빠른 속도로 진행되고 있는데 비하여 노인보건제도나 노인 간병요양원 시설은 제대로 개발되어 있지 않다. 본 연구는 정부가 구상 중에 있는 노인건강관리법(가칭) 시안과 관련하여 ① 노인종합병원, 노인간병요양원 제도 개발의 타당성을 규명하고, ② 이들 시설의 수요를 예측하며, ③ 소요인력의 구성에 대하여 검토하고, ④ 노인의료보험 급여의 범위와 재원조달 방법에 대하여 분석하였다. 본 연구에서는 1997년에 노인진료비가 l조원을 상회하고, 최근 3년간 노인진료비의 연평균 증가율이 22.9%를 상회하고 있어서, 이러한 초고속 증가에 대응하자면 보건소를 중심으로 하는 공공의료에서의 노인보건사업 강화 및 가정보건(방문간호) 사업의 개발이 절실함을 보여주고 있다. 본 연구의 추적결과 2000년에는 8,137개의 노인병원 병상과 24,411개의 노인간병 요양원 병상이 필요한 것으로 드러났다. 의료보험의 노인의료비는 총 진료비의 12%이다. 현재는 의료보험 노인의료비 공동사업으로써 노인진료비를 조달하고 있지만, 장차 이 비율이 20%-25%에 접근하게 되면 현행 방식보다는 별도의 노인의료보험제도를 개발하든지 지방자치단체의 재정투입이 필요할 것으로 보았으며, 이에 대한 계속적인 연구검토가 필요하다. 그리고 미래의 노인보건의료체계를 구축하기 위해서는 공공부문과 사적부문간의 역할 분담이 필요하고 상호보완적 관계를 유지하도록 해야 할 것이다.

영문 초록

It is widely acknowledged that health policies on geriatric health care have poorly been developed in Korea. The objective of this study is to examine the feasibility of the Geriatric Health Management Law, currently planned to introduce by the government. More specifically, it has attempted to predict the demand for beds of geriatric hospitals and nursing home facilities, and to analyze the possibility of implementing an independent health insurance for the aged. The study estimates that the medical care expenditures for the aged under the current health insurance program will exceed a thousand billion won by 1997 and the average increase rate of geriatric care has fluctuated around 22.9% for the last three years. Based upon these findings and foreign experiences, this study has predicted that Korea would need 8,137 hospital beds and 24,411 nursing home beds by the year 2000. Home health care programs for the aged are strongly recommended. In addition, the study has emphasized that the current Health Insurance Coordination Fund for the aged would be insufficient to meet health expenditure and then Korea would be required to set up another financing measures such as the health insurance program for the aged and increased financial inputs by local government. It is a necessity to coordinate roles of the private sector and of the public sector, and to maintain complementary relationship each other in the future geriatric health care system.
