구강기능훈련이 뇌성마비 아동의 발화 산출에 미치는 효과

국문 초록

본 연구는 발화가 거의 되지 않는 뇌성마비 아동(6;8) 세 명을 대상으로 구강기능훈련을 실시하고, 구강기능훈련이 대상의 발화 향상에 효과가 있는지 알아보고자 하였다. 중재는 관련 자료를 검토하여 구성하였으며, 주당 3회기씩 8주, 회기 당 약 25~30분을 기준으로 총 24회기로 ABA 설계로 실시하였다. 구강기능훈련의 효과를 검증하기 위해 기초선, 중재 및 유지 단계별로 구강기능검사와 모음 및 자음 따라 말하기 과제를 실시하고 평균 구강감각 기능점수, 발화 가능한 모음 및 자음 수를 산출하여 단계별로 비교하였다. 결과는 첫째, 평균 구강감각 기능점수와 발화 가능한 모음 수는 기초선 단계에 비해 중재 및 유지 단계에서 계속 증가하였다. 그러나 발화 가능한 자음 수는 기초선에 비해 중재 단계에서는 증가하였으나 유지 단계에서는 감소하였다. 논의 결과, 구강기능훈련은 뇌성마비 아동의 구강감각 기능 및 모음과 자음의 발화를 향상시키는 데 효과적이라고 할 수 있었다. 그러나 단기간의 구강기능훈련은 산출된 자음의 발화를 유지시키는 효과는 적은 것으로 분석되었다.

영문 초록

The purpose of this study was to examined whether the oral function training was effective in improving the speech production of children with cerebral palsy. The subject participated in the study is 3 children, they have mean aged 6 years and 8 months with cerebral palsy. This study is consisted of 3 phases; baseline, intervention, and maintenance(post) stage. It was carried out for a total of 24 sessions. 25~30 minute oral function training sessions were provided 3 times a week for 8 weeks. To verify the effectiveness of the oral function training, the oral sensory function score, the number of utterable vowels and consonants were repeatedly measured according to stage, and the average score was compared step by step. The following results were obtained. First, the average oral sensory function score increased to 1.45 in the baseline stage, 4.08 in the intervention stage, and 6.00 in the post evaluation. Second, the total number of utterable vowels was 1.56 in the baseline stage, but increased to 4.08 in the intervention stage, and 5.67 in the post-stage. Third, the number of utterable consonants was 1.56 in the baseline step, and increased to 4.58 in the intervention stage. but decreased slightly to 3.11 in the post stage. The results suggest that, oral function training had an effective in the improving oral sensory function, which is a basic function for improving speech in children with cerebral palsy. It is also effective in prompting the ability to begin to speak vowels or consonants. However, the effect of maintaining the utterance of the consonants was found to be small, indicating that continuous oral function training is necessary to promote the consonant utterance ability of children with cerebral palsy.
