간호대학생의 환자안전사고 관찰 경험과 사고 공개에 대한 태도 및 보고 의도와의 관계

국문 초록

본 연구는 간호대학생이 임상실습 중 환자안전사고를 관찰한 경험 여부에 따라 환자안전사고 공개에 대한 태도 및 환자안전사고 보고 의도에 유의한 차이가 있는지 확인하고, 또한 환자안전사고 관찰 경험 횟수와 환자안전사고공개에 대한 태도 및 환자안전사고 보고 의도와의 관계를 확인하기 위하여 수행되었다. 연구 대상자는 간호 교육과정에 환자안전 관련 과목이 정식으로 개설되어 있지 않은 3개 간호대학을 편의표집하여 총 215명의 대상자로부터 수집된 설문지 응답을 분석하였다. 연구결과 45.1%에 해당하는 97명의 학생들이 임상실습 중 환자안전사고 관찰 경험이 있는 것으로 나타났다. 환자안전사고 관찰 경험 유무에 따른 사고 공개 태도와 사고 보고 의도를 비교한 결과 환자안전사고가 공개되어야 한다는 태도에는 유의한 차이가 없었으나(t=0.438, p=.662), 사고 보고 의도에 대해서는 환자안전사고 관찰 경험이 있는 학생들이 유의하게 낮은 것으로 확인되었다(t=-2.547, p=.012). 환자안전사고 관찰 경험횟수는 환자안전사고 공개 태도 및 보고의도와 상관관계가 없는 것으로 확인되었으며, 환자안전사고 공개 태도와 보고 의도는 정적인 상관관계가 있는 것으로 확인되었다(r=.144, p<.05). 이러한 결과는 간호대학생들이 환자안전사고가 공개되어야 한다는 태도를 갖고 있음에도 불구하고, 임상실습 도중 환자안전사고를 관찰한 경험이 환자안전사고 보고 의도에 부정적인 영향을 미칠 수 있음을 시사하고 있다. 따라서 간호대학생들이 환자안전사고 보고의 중요성을 이해하고, 사고 보고 후 긍정적 피드백에 대한 경험을 가질 수 있도록 하기 위한 임상실습 지도 교육 전략을 마련할 필요가 있을 것이다. 이를 통해 간호대학생이 환자안전사고에 대하여 시스템을 개선시킬 수 있는 학습의 기회로 인식하고 바람직한 환자안전문화를 형성하도록 하여 추후 간호사가 되어 환자안전을 향상시키는 데 기여할 것으로 기대된다.

영문 초록

The purpose of this study is to identify significant differences in attitude toward patient safety incidents disclosure and intention to report patient safety incidents depending on whether nursing students had experienced observing patient safety incidents during their clinical practicum. Moreover the relationship among the number of experience of observing patient safety incidents, attitude toward patient safety incidents disclosure, and intention to report patient safety incidents was investigated. A cross sectional descriptive design was used with convenience sampling. Fourth-year nursing students (n=215) from three nursing school completed the survey. A self-report questionnaire was used to assess experiences of observing patient safety incidents, attitude toward patient safety incidents disclosure, and intention to report patient safety incidents. Nearly half(45.1%) of the nursing students 97 students have experienced observing patient safety incidents during clinical practicum. The result indicated that there was no significant difference in attitude toward patient safety incidents disclosure, while respondents had experienced observing patient safety incidents reported lower intention to report patients safety incidents comparing to respondents who had no experience of observing patient safety incidents. The number of experience observing patient safety incidents was not signifiantly related to attitude toward patient safety incidents disclosure and intention to report patient safety incidents. A positive correlation was observed between the attitude toward patient safety incidents disclosure and intention to report patient safety incidents. The result of this study suggests that the experience of observing patient safety incidents during clinical practicum for nursing students could have a negative effect on their intention to report patient safety incidents, despite the nursing students agree with that patient safety incidents should be disclosed. Therefore, educational strategies are needed for nursing students, which are to understand the importance of patient safety incidents reporting and to be given positive feedback after incidents occurring. Through the starategy, nursing students can perceive patient safety accidents as a learning opportunity to correct the system problem and imrove their patient safety culture, which is expected to contribute to improving patient safety as nurses in the future.
