국민건강보험 가입에 영향을 미치는 요인분석 연구

국문 초록

최근 한국의 보건의료 환경은 대외적으로 한-미 FTA협정을 통한 의료서비스시장 개방과 국내적으로는 영리병원 허용 논란 등으로 현행 국민건강보험제도의 존폐를 우려하는 목소리가 점점 확대되고 있다. 본 연구에서는 의료서비스 이용에서 가장 우선시 되어야 할 국민건강보험 가입에 영향을 미치는 요인에 대해 알아보기로 한다. 연구모형의 종속변수는 국민건강보험가입여부이며, 독립변수에서 인구사회학적 요인은 성별, 연령, 배우자 유무, 교육수준, 가구원수이고, 경제학적 요인은 경제활동수준, 가구소득 수준, 가구형태, 노인가구원 유무이다. 통제변수로는 의료필요 요인인 주관적 건강 상태와 장애가구원 비중, 만성질환 가구원 비중으로 하였다. 분석 자료는 2006년부터 2009년까지 4개년 간 조사된 한국복지패널조사 패널데이터를 이용하였으며, 이항로지스틱 회귀분석결과 의료서비스 이용자의 인구사회학적 요인, 경제학적 요인, 의료필요 요인이 국민건강보험가입에 유의적인 영향력이 존재하는 것으로 분석되었다. 국민의료보장제도가 시행되고 있는 한국에서 우리 관계 당국이 해결해야 할 과제가 있음을 말해주고 있으며, 가장 시급하게는 인구사회학적 요인과 경제학적 요인으로 인해 수입이 없거나 경제활동을 할 수 없어 장기간 연체된 국민건강보험료를 일시불로 납부할 수 없는 노숙자, 독거노인, 저소득층등 의료서비스 이용의 사각지대에 놓여있는 빈곤층에 대한 해결책이 시급하다 하겠다.

영문 초록

In this research, the factors affecting the join rate for the national health service were decided to be analyzed. Whether the public medical users’ joined the national health service or not was set as the dependent variable. The independent variables were classified into two categories, sociology of population factors and economic factors. The control variables include the medical need factors. The result of the binomial logistic regression shows that the public medical service users’ sociology of population factors, economic factors and medical necessary factors have significant effects on the joining-rate for the national health service. This bespeaks that the relevant authorities in Korea, where the whole nation medical social security is being enforced, have a problem to solve. Utmost urgently, people who lack income or are not able to be in economic activity due to sociology of population factors and economic factors, are consequently in long-time overdue with their national health service insurance premiums such as homeless people, senior citizens who live alone and the low-income bracket, who are in the blind spots of the medical service, must be considered to solve this problem.
