2차 병원 간호사의 측위와 하지혈압 측정실태 분석

국문 초록

본 연구는 혈압측정시 측위로 있을 수밖에 없는 환자나 혈압측정을 하지에서 할 수밖에 없는 자에 대한 간호사들의 혈압측정 실태 및 측정시의 정수압과 측정부위 둘레의 영향에 대한 간호사의 인지정도를 분석하였다. 281명의 2차 병원 간호사를 대상으로 분석한 결과 환자가 측위일 경우 사지 중 정수압이 가장 낮은, 침대에 면하지 않은 위쪽 팔에서 혈압을 측정하는 자가 가장 많았고 하지 혈압측정은 92.9%가 커프너비가 좁은 상박용 혈압계를 이용하였으며 그 중 57.1%는 슬와동맥을, 42.9%는 후경골 동맥을 이용하였다. 연구대상 간호사의 91.8%는 혈압측정시 정수압 및 측정사지둘레와 커프의 너비는 인지하지 않았다. 혈압은 정수압 및 사지둘레에 대한 혈압기 커프너비의 영향을 받지만 임상현장에서 간호사들의 혈압값 측정은 이를 간과하고 있는 것으로 나타났고 이는 대상 간호사들의 특성에 따른 차이가 없었다. 결론적으로, 임상에서 흔한 측위와 하지에서의 정확한 혈압 측정값 산출과 측정자별 편차를 최소화하기 위한 혈압측정 지침개발 및 임상현장교육이 필요하다고 본다.

영문 초록

Purpose: The purpose of this study was to identify the realities of clinical nurses’ blood-pressure measuring sites; either in lateral position or in lower extremities which some clients’ blood pressure cannot be measured except for and apply exact measurements of blood pressure to the practical use. Method: Nurses(N=281) were recruited in a metropolitan city. The data were collected by self-reported questionnaires from April to June 2007 and analyzed with descriptive statistics and chi-square test. Results: In the right lateral position, 49.8% of participants’ blood pressure measurements were done on their left upper arms, and in the left lateral position so were 48.0% of participants’ measurements on their right upper arms. Also, 92.7% of participants’ blood pressures were measured in the lower extremities with the small available cuff and bladder. In measuring blood pressure depending on client’s characteristics, there was no significant difference in cognizing the effect of hydrostatic pressure and extremities’ girth. Conclusion: Hydrostatic effects can be the cause of dropping blood pressure of upper arms in the lateral position. This study suggest that several studies should be performed to test the influence of hydrostatic pressure and extremities’ girth in blood pressure measuring.
