의무기록과 건강보험청구시 주 진단명 불일치를 유발하는 요인 연구

국문 초록

본 연구는 일개 대학병원의 2008년 퇴원환자 총 30,793명 중, 4월과 9월 퇴원환자 3,056명을 조사대상으로, 의무기록과 건강보험청구 시 각각의 주진단명이 서로 일치하지 않는 원인을 파악하고자 한다. 연구 결과 주 진단명 상위 5위 중 일치율에 영향을 미치는 요인으로 협심증(I20), 감염성 및 상세불명의 위장염(A09), 세균성 폐렴(J15) 및 바이러스성 폐렴(J12) 등 모든 질환군에서 유의한 차이를 보였으며(p=0.000), 모델에 대한 설명력은 37.2%로 나타났다. 이와 같은 결과로 볼 때, 의무기록상 주진단명과 건강보험청구 시 주진단명을 일치시켜야 보다 정확한 국가 보건통계 및 국민건강 지표로 활용할 수 있을 것이다. 그러므로 의무기록과 건강보험청구 시의 주진단명을 서로 일치시킬 수 있는 명확한 지침과 질병분류자들에 대한 지속적인 교육이 필요할 것으로 사료된다.

영문 초록

The purpose of this survey was to find the factors affecting that principal diagnosis disagreement between medical records and input in health insurance claims program. The subject was 3,056 people who discharged patients at an university hospital from May to September, 2008. Five high rank of diagnosis effected on agreement rate that were angina pectoris, bacterial and viral pneumonia, gastroenteritis of infectious and unspecified diagnosis etc. All the group of diagnosis revealed significant differences. The explanation for the Model was 37.2%. The agreement diagnosis between medical records and input in health insurance claims program was very important. That made used of national health statistical, national health indicators and national health plan. and That was suggested the guideline to unify the diagnosis between the two.
