산부인과 진료의뢰서의 추정 진단명과 최종 진단명의 일치율에 관한 연구

국문 초록

2003년 1월 1일부터 1년간 1, 2차 의료기관에서 본 대학병원 산부인과로 의뢰되어 입원한 환자 505명을 조사대상으로 했다. 이번 연구는 1, 2차 의료기관에서 의뢰된 산부인과 환자를 대상으로 의무기록정보에 의한 주 진단명과 퇴원시의 주 진단명의 일치율을 알아본 결과, 1, 2차 의료기관에서의 진료의뢰서 진단명과 3차 진료기관의 최종진단명은 대부분 일치하였으나 타과에서 의뢰된 경우, 하급 의료기관(보건소, 산부인과 전문의가 없는 개인의원 등)에서 의뢰된 경우, 환자나 보호자의 요구에 의한 의뢰서, 그리고 병리학적 검사가 시행되지 않은 의뢰서의 경우는 일치하지 않는 경우가 많았다. 따라서 이러한 경우에 발생하는 의료분쟁을 피하기 위해서는 철저한 병력 청취 및 이학적 검사가 필요할 것으로 파악되었다.

영문 초록

Study subjects were 505 patients who were hospitalized in the department of obstetrics and gynecology, medical center of one university, being referred from the 1st or 2nd medical institute during 1 year from January 1, 2003. We compared the concordance rate between the principal diagnosis on the written request made at 1st, 2nd hospital using the 3rd revision of Korea standard disease cause of death classification, the 10th revision of international disease classification system and the final diagnosis inputting to the hospital computing network by doctor at the time of leaving hospital. Mainly, principal diagnosis on the written request of 1st, 2nd medical institute and the final diagnosis of the 3rd medical center was consistent. But they were inconsistent when patients were referred from other department of medicine or lower class medical institution, in the cases of the written request by patients or their protectors and in the absence of pathological tests.
