‘마마’와 ‘호열자’로 보는 개항기 보건의료

국문 초록

1876년 개항과 함께 조선은 새로운 국제질서에 편입되어 제국주의 열강과 경쟁관계에 서게 되었다. 조선 정부는 부국강병을 위한 유력한 수단으로 국가 차원에서의 인구의 양적ㆍ질적 관리에 큰 관심을 가졌다. 이에 따라 조선의 국가보건의료체계는 획기적이고 선진적인 방향으로 재편되어 갔다. 갑오개혁을 거치면서 비로소 근대적인 보건의료체계가 제도적으로 확립되기 시작했고, 그 과정에서 오랜 시간과 수많은 시행착오가 뒤따랐다. 그래서 개항 이후부터 갑오개혁 시기(1876~1894)까지 근대적 보건의료가 도입되는 과정에 주목하였다. 두창과 콜레라는 근대 의료, 보건ㆍ위 생, 방역 등이 이 땅에 어떻게 도입되고 시행되는지를 살펴보는 가장 좋은 예시이다. 두창과 콜레라에 대한 근대적 접근이 제일 처음 시도된 곳이 부산이었다. 이 글에서는 개항기 부산에서 행해졌던 근대보건의료, 그 중에서도 두창과 콜레라 방역을 중심으로 살펴보았다. 구체적으로는 부산에서 처음 실시되었던 우두접종의 모습을 최초의 근 대식 의료시설인 제생의원과 「종두조례」를 통해 살펴보았다. 또한 최초로 설치된 전염병전문 격리치료시설인 ‘절영도 소독소 및 부산피병원’이 설치되는 과정도 정리해 보았다. 이 글은 그동안 잘 알져지지 않고 주목 받지도 못한 개항기 부산지역의 보건의료 실태를 정리하는 데 일차적인 목적이 있다. 우리나라에 근대적 보건의료체제가 도입되는 초기 양태를 보다 구체적으로 그려볼 수 있을 것이다.

영문 초록

Joseon opened in 1876 and was forcibly incorporated into a new international order. it is in competition with the imperialist powers. Joseon government was very interested in the quantitative and qualitative management of the population at the national level, because it is a prominent instrument for wealthy nations. As a result, the health care system of Joseon has been restructured in a dramatic and advanced direction. It has taken a long time and had many trial and error before establishing the national health care system of Joseon. After Gabo reform in 1894, a modern health care system began to be institutionally founded. Therefore, this essay pay attention to the change process of modern health care system from the opening of port to the time before Gabo reform(1876~1894). Pox and Cholera are the best examples of how modern medicine, health, hygiene and sanitation are introduced and implemented in joseon. Busan was the first place where the modern approach to pox and cholera was attempted. In this essay, I review the modern health care system in Busan, concretely focusing on the prevention of pox and cholera. Especially, I review the appearance of vaccination that was first performed in Busan, through the exemples of the Jongdujolye(種痘條例) and Jesaeng-hospital(濟生醫院) that is first modern medical clinic in Joseon. In this essay, I also summarized the process in founding the first infectious disease specialized medical facility, ‘Jeol-yeongdo Disinfection facility(絶影島消毒所) and Busan Evacuation hospital(釜山避病院)’. This essay is primarily aimed at summarizing the health care situation in Busan, which has not been well known and has not been noticed yet. Through this essay, I also will be able to examine more precisely the initial aspects of introducing modern health care system in Korea.
