국문 초록

배경 및 목적: 음절성 발성은 실제 단어와 유사하며 이후 말, 언어발달과 관련이 있다. 본 연구는 9-24개월까지 구개열, 일반 영유아의 말, 언어발달을 종단적으로 살펴보고 초기 발성과 구어 간 관계를 살펴보았다. 방법: 구개열 아동 8명과 일반 아동 9명이 연구에 참여하였다. 생후 9, 15, 24개월에 수집된 음성 자료를 토대로 발성 및 말소리 산출 특성을 나타내는 객관적인 측정치를 구해 집단과 월령에 따라 비교하였다. 구개열 수술 전, 후의 언어이전기 발성과 24개월 발화 간 상관을 살펴보았다. 결과: 구개열 아동은 수술 후 전음절성 발성이 줄고 음절유형이 증가하였으나 일반 아동에 비해 자음목록이 제한적이었고 24개월에는 구강 압력 자음으로 시작하는 어휘, 자음목록 수, 정확성이 낮았다. 9개월에 음절유형이 다양한 구개열 아동은 24개월에 자음목록과 어휘 수가 많았으며 전음절성 발성이 많을수록 구어 단계 조음 정확성이 낮았다. 수술 후 발성의 음절유형 목록, 음절성 발성 산출률, 종성 자음이 많을수록 표현어휘가 많았고 초성 자음이 많을수록 조음 정확성이 높았다. 논의 및 결론: 본 연구결과는 구개열 아동의 발성 및 및 초기 말, 언어 발달에 대한 정보를 제공하며, 말, 언어문제를 보이는 구개열 아동을 조기에 선별할 수 있는 근거자료를 제시한다.

영문 초록

Objectives: Syllabic vocalization is closely related to the development of verbal language. This study aims to longitudinally examine the development of vocalization, phonology and vocabulary in children with and without cleft palate (CP). The study also investigates the relationship between vocalization and later spoken language. Methods: The partici-pants were 8 children with CP and 9 typically developing children. Spontaneous utterances collected at 9 and 15 months of age were analyzed regarding vocalization, word produc-tion ratios, length, structure, and phonological characteristics of vocalizations. We also ana-lyzed expressive vocabulary and phonological characteristics based on data at 24 months. The correlation between verbal stage utterances and vocalizations prior to and following palatal repair was also examined. Results: Children with CP showed a smaller consonant inventory than children without CP. Children with CP indicated persistent increases in syl-labic vocalizations from 9 to 15 months but still showed delays compared to their noncleft peers. Children with CP have a limited inventory of vocabulary starting with oral pressure consonants, a limited oral pressure consonant inventory, and limited accuracy of initial oral pressure consonants at 24 months of age. Children with CP showed a close relationship between early vocalization at 9 and 15 months and spoken language at 24 months. Con-clusion: Children with CP have deficits in vocal developments, which appear to persist fol-lowing palate closure and involve restrictions on subsequent speech and lexical acquisi-tion. The study supports the role of vocalization as an important predictor of later speech and language performance.
