치매노인 가족의 간병 경험에 관한 선험적 현상학 연구

국문 초록

본 선험적 현상학 연구(transcendental phenomenological study)는 알츠하이머성 치매 또는 혈관성 치매 노인을 돌보는 주 가족 보호자의 경험을 그들의 시각에서 이해함이 목적이다. 가족보호자 10명의 심층 인터뷰 및 연구자의 연구노트-연구일기와 필드노트-를 통해 얻은 연구 자료는 Moustakas(1994)에 의해 수정된 Van Kaam의 현상학 분석방법으로 분석하였다. 참여자 모두 각자의 고유한 경험을 하지만 이들 가족보호자의 경험을 특징짓는 핵심테마는 다음의 여섯 가지로 요약할 수 있다. 가족보호자들은 (1) 알츠하이머성 치매를 질병으로 인정함으로써 치매환자의 상태를 이해하려고 노력하지만 치매로 인한 환자의 변화를 힘들어하며, (2) 보살핌에 대한 개인 특유의 철학을 바탕으로 치매환자 간병에 대한 나름대로의 정의를 세우고 있으며, (3) 치매환자를 돌보면서 인간의 실존, 삶과 죽음에 대해 숙고하게 되고, (4) 치매환자와 자신의 처지에 대해 복잡한 심정으로 괴로워하면서, (5) 노부모 공양에 대한 세대차이가 있음을 주지하며 노후대책과 장기요양보험의 중요성을 느끼고, (6) 치매환자를 위한 의료 및 사회복지 서비스를 적극적으로 이용하면서 드는 생각을 나누고 싶어 한다. 연구결과는 노인 가족원이 치매증상을 보이기 시작할 때, 치매진단을 받기 전후의 상황, 환자의 치매증상이 심해지면서 환자를 본격적으로 돌보게 되는 과도기적 경험, 그리고 간병을 둘러싼 가족 내 긴장과 갈등의 상황에 대한 이해를 높인다. 또한 환자의 사후에 가족보호자가 겪는 경험은 어떠한지도 알 수 있다. 본 연구 결과를 바탕으로 치매환자를 돌보는 가족을 위한 다학문적 노인복지 실천을 논한다.

영문 초록

This transcendental phenomenological study was conducted in an effort to understanding of the experiences of caregiving from the perspective of caregivers in Korean-American families. Interviews with ten primary caregivers of elderly patients with Alzheimer's disease and vascular dementia were analyzed with the systematic procedure of Moustakas'(1994) modification of the Van Kaam method. The findings revealed that caregiving had different meanings for each caregiver and the meaning of the experience changed with the progression of the disease within each individual patient. However, there were six core themes that characterized these caregivers' experiences: (1) understanding about elderly patients with AD by accepting AD as a disease, (2) self-definition & self-appraisal of caregiving, (3) contemplating existential thoughts, (4) polarity of ambivalent feelings, (5) questioning of the transgenerational continuity of filial piety, and (6) different experiences with the formal care system. The findings unveiled how caregivers and families coped with the transition into caregiving both before and after the onset of dementia as well as what caregivers experienced after their patients died, which is useful in developing a stage-specific systematic intervention for family caregivers. Implications for interdisciplinary practice for families with dementia are addressed.
