재가노인복지서비스 유형별 치매노인 주부양자의 부양부담 완화 효과

국문 초록

본 연구는 재가노인복지서비스 유형별로 치매노인 주부양자의 부양부담 완화효과가 시간적 흐름에 따라 어떠한 차이를 보이는지 실증적으로 인과관계를 살펴보았다. 인과관계를 검토하기 위하여 치매노인 주부양자를 대상으로 공식적인 사회적 지지 형태인 재가노인복지서비스를 이용하는 경우가 이용하지 않는 경우보다 부양부담 완화효과가 있는지 집단 간 비교하였다. 이를 위하여 유사 실험설계방법인 비동일 통제집단 설계방법을 활용하여 실험집단을 치매 가정봉사원파견서비스, 치매 주간보호서비스, 치매 단기보호서비스로, 통제집단을 가정에서 케어로 설정하여 사후조사와 회고적 사전조사(retrospective pretests)를 실시하였다. 재가노인복지서비스 유형별로 치매노인, 치매노인 주부양자의 인구사회학적 특성이 차이가 있는지 살펴보기 위하여 종속변수가 비연속 변수인 경우 카이 스퀘어 검정, 연속변수인 경우 독립표본 티검정(Independent Samples t-test), 일원배치분산분석(ANOVA)을 활용하였다. 분석결과를 토대로 집단 간 이질적인 특성으로 파악되는 변수(치매노인의 성별, 인지기능 및 문제행동, 치매노인 주부양자 성별, 주부양자 생활정도, 부양기간, 비공식적 사회적지지)와 회고적 사전부양부담은 집단 간 부양부담 완화효과를 살펴보는 공변량 분석에서 통계적 통제를 실시하였다. 공변량 분석결과를 토대로 3개의 가설을 검증한 결과, 3개의 가설 중 치매 주간보호서비스와 가정 케어의 부양부담에 대한 비교에 대한 가설만 통계적으로 유의미한 결과로 나타났고, 치매 가정봉사원파견서비스와 가정에서 케어, 치매 단기보호 서비스와 가정에서 케어는 통계적으로 부양부담 완화효과가 없는 것으로 파악되었다.

영문 초록

This paper is to identify the cause and effect by experiment designs that primary care-giver's burden for senile dementia elderly could reduce the care burden as for the lapse of time. To examine the cause and effect, among the primary care-giver's burden for senile dementia elderly, it compared between groups with formal social support and tried to see whether buffering effect for care burden less in not using community-based service than others. Using non-equivalent control group designs, quasi-experimental designs, it set up experimental group as home help service for senile dementia elderly, day care service for senile dementia elderly, short stay for senile dementia elderly and control group as home care for senile dementia elderly. And then it did retrospective pretests and post tests based on them. This paper proved three hypotheses. As the proof results of hypothesis based on the co-variance analysis, one research hypothesis among three were chosen the significance level and the rest was dismissed. Chosen research hypothesis is burden comparison between day care service for senile dementia elderly and care burden at home. While home help service for senile dementia elderly and care burden at home, short stay service for senile dementia elderly and care burden at home could not proved the significance level on buffering effect.
