열성 경련에 의한 간질지속상태 환아의 임상적 특성과 예후

국문 초록

목 적 : 열성 경련은 소아에게 가장 흔한 경련성 질환으로서 일반적으로 양성 경과를 취하는 것으로 알려져 있으나 간질지속상태에 이르렀을 때 급성 뇌손상이나 신경학적 후유증의 유발 여부와 향후 내측두엽 경화증이나 해마 위축을 일으켜 난치성 측두엽 간질의 원인이 될 수 있는 지에 대해서는 논란이 있는 상태이다. 도한 열성 간질지속상태 환아에 대한 장기적인 항경련제의 투약 여부에 대해서도 명확한 지침은 없는 상태이다. 이에 저자 등은 열성 경련 환아 중 열성 간질지속상태를 보인 환아의 임상적 특성과 예후를 알아보고자 본 연구를 시행하였다. 방 법 : 1997년 2월부터 2001년 2월까지 한림대학교 강남성심병원 소아과에서 열성 경련으로 입원하여 치료한 환아 중 30분 이상의 경련발작을 일으킨 환아 20명을 열성 간질지속상태군으로 하였고 같은 기간 중 처음 열성 경련을 일으켜 입원 치료하였던 환아 153명을 대조군으로 하였다. 대상환아의 병록지를 후향적으로 검토하여 진단 당시의 임상 양상을 고찰하였고 추적 관찰은 퇴원 후의 병록지를 검토하였으며 이 중 추적 관찰ㄹ이 어려운 경우는 전화 문답으로 조사하였다. 결 과 : 열성 경련군과 열성 간질지속상태군의 임상적 특성 비료에서 원인 질환, 발병 연령, 신경학적 이상 과거력, 열성 경련 가족력, 간질 가족력, 경련 형태, 발열 기간 및 정도에 있어 두 군 사이에 통계학적으로 유의한 차이를 보이지 않았다. 뇌자기공명영상 검사상 두 군 모두 특이 소견을 보이지 않았고 뇌파 검사상 두 군데에 각각 1례씩의 비정상소견을 보여 통계학적으로 유의한 차이를 보이지 않았다(P=0.46). 예후의 조사에서 열성 간질지속상태군 환아 20례 모두에서 사망한 경우는 발견되지 않았으나 1례에서 우측 팔ㄹ의 근력 저하를 발견하였고 열성 경련군 환아 153례 모두 사망하거나 인지기능, 운동능력의 손상을 보이는 경우를 발견할 수 없었다. 결 론 : 본 연구에서 열성 경련군과 열성 간질지속상태 환아군 사이에 임상적 특성이나 예후에 있어 차이가 없었으나 열성 간질지속상태 환아군의 수가 적고 추적관찰기간이 길지 못한 것이 원인일 수 있으며, 또한 열성 경련과 난치성 측두엽 간질과의 연관성에 관한 연구를 위해서는 향후 좀더 광범위하고 장기적인 추적관찰을 통한 연구가 필요할 것으로 사료된다.

영문 초록

Purpose : Febrile seizure(FS) is the most common form of epileptic syndrome in pediatrics and known to have benign clinical course. In regard of febrile status epilep-ticus, however, there remails controversy about whether acute cerebral injury or neuro-logical deficit can ensue and whether it can ultimately lead to intractable medial tem-poral lobe epilepsy secondary to mesial temporal sclerosis and hippocampal atrophy. We performed this study to determine the general clinical characteristics and prognoses in patients with febrile status epilepticus(FSE). Methods : The FSE group was 20 children who developed status epilepticus after FS and admitted to the department of Pediatrics, Kang-Nam Sacred Heart hospital, Hallym university, form February, 1997 to February, 2001, while the FS group was 153 children who were admitted for the first episode of FS at the same period. We retrospectively analyzed the patients' charts for the clinical status at the time of diagnosis and in the follow-up period after discharge. In case of follow-up loss, we tried to contact the family by telephone. Results : We found no statistically singnificant difference in etiology, patients' age, prior history of neurological abnormality, family history of epilepsy, seizure type, duration and degree of fever between the two groups. MRI scannings showed negative results of the all patients. Only one patient from each group shwed abnormality in EEG, which was not statistically significant. In the analysis after the diagnosis, no death and one patient with mild weakness in the right arm was found among the FSE group. Among 153 patients of FS group, no death or postictal decline in cognitive and motor function was found. Conclusion : We found no significant differences in clinical characteristics and prog-nosis between the FSE and FS group, but small sample size and short follow up period necessitates further study with more patients and longer follow up in order to verify the relationship between the FS and intractable temporal lobe epilepsy
