무균성 수막염에 동반된 Ramsay Hunt 증후군 1례

국문 초록

RHS는 VZV의 재활성화에 의한 일종의 슬 신경절염을 유발하여 일측성 안면 마비와 함께 외이도, 이개, 구강, 점막 등의 수포성 발진, 이통 등을 초래하는 질환으로, 소아에서는 매우드물게 보고 되고 있다. 소아에서는 성인에 비하여 특징적인 수포성 발진의 발현이 다소 지연되어 나타날 수 있고 이로 인하여 첫 진단시에 Bell 마비로 오인하게 되거나 RHS의 진단과 치료가 늦어질 수 있다. 다라서 안면 마비를 주소로 내원하는 소아 환자의 진단에 있어서 RHS의 가능성도 항상 인식하고 대상포진형 발진이 발현하는지 신중하게 관찰해야 할 것으로 사료된다. 저자들은 이통과 동측의 안면 마비를 주소로 내원한 10년 8개월된 남아에서 특징적인 임상 양상과 VZV 항체 측정, 뇌척수액 검사등을 통하여 확진하고 acyclovir와 prednisolone으로 치료하였던 무균성 수막염이 동반된 RHS 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

영문 초록

Ramsay Hunt syndrome(RHS or herpes zoster oticus) is caused by reactivation of la-tent varicella-zoster virus(VZV) in the geniculate ganglion of the seventh cranial nerve. Unilateral facial paralysis is accompanied by herpetic vesicles on the external auditory canal or in the mouth. The prognosis is not good as that of idiopathic facial palsy. Ag-gressive treatment with acyclovir, in combination with steroids, is recommended. RHS is thought to affect adults primarily, but a few cases of children with RHS have been reported. We present a case of RHS conbined with aseptic meningitis in a previously healthy 10-year-8-month-old boy. He showed a complete peripheral facial palsy on the right side with CSF pleocytosis. Eight days after the onset of the facial palsy, the char-acteristic herpetic vesicles appeared on the pinna of the right side. The analysis of sera confirmed the clinical diagnosis of RHS with a positive IgG and IgM-ELISA antibody titer of VZV. Although we administered acyclovir and prednisolone adequately to the pa-tient, he has shown an incomplete recovery of the facial palsy on a follow-up visit. The physicians should be prudent in the diagnosis of idiopathic facial palsy or RHS, and must watch for the appearance of vesicles in children with facial palsy.
