아급성 경화성 범뇌염(Subacute Sclerosing Panencephalitis) : 임상 경험 6례

국문 초록

목 적 : 아급성 경화성 범뇌염(subacute sclero-sing panencephalitis, SSPE)은 10세 전후의 청소년기에 발생하여 1-3년의 임상 경과를 거쳐 사망하게 되는 심한 퇴행성 신경질환이다. 홍역 감염 후 6-8년의 잠복기르르 거쳐 발병하는데, 2세 이전에 홍역을 앓을 경우 위험성이 증가한다. 국내에서 1-2년 전 홍역이 전국적으로 유행하였고 특히 2세 미만의 환아 비율이 높아 4-6년 후 SSPE 환자가 많이 발생할 가능성이 있다. 저자들은 SSPE 6례의 임상적 고찰을 통해 조기 진단에 도움이 되고자 하였다. 방 법 : SSPE 환자 6명(남 4, 여 2)을 대상으로 하였다. 후향적으로 임상 소견, 뇌척수액 검사 소견, 뇌파 소견 및 뇌 자기공명영상 소견 등을 정리하였다. 결 과 : 6례 중 4례가 1세 이전에 홍역 감염이 있었다. 임상 증상은 근간대 6례, 넘어짐 4례, 운동 실조 4례, 구음장애 3례, 발작 2례, 사지의 불수의 운동 2례, 진전 2례, 고개 떨굼 1례, 수면장애 1례 등이었다. 검사가 시행된 전례에서 CSF IgG, CSF IgG/albumin ql, CSF/serum IgG index가 상승되어 있었고 oligoclonal band도 양성이었다. 뇌척수액 항홍역 바이러스 IgG 항체는 전례에서 양성이었다. 뇌파상 전례에서 고진폭 δ파가 전두부에서 관찰되었고, 다초점성의 간질파도 주로 전두부에서 관찰되었다. 뇌 자기공명영상상 T2 강조영상에서 심부 백질을 중심으로 고신호강도를 보였다. 결 론 : SSPE는 전형적인 임상양상과 뇌척수액 항홍역 바이러스 항체 역가의 상승으로 진단할 수 있고 뇌파 소견, 뇌척수액 검사 소견, 그리고 뇌자기공명영상 소견이 진단에 도움이 될 수 있다. 향후 국내 SSPE의 발생률이 증가할 위험성이 높으므로 적극적인 진단 및 치료를 요할 것이다.

영문 초록

Purpose : Subacute sclerosing panencephalitis(SSPE) is a severe and usually fatal neurodegenerative disorder of childhood and adolescence. The etiology is related to previ-ous measles infection especially during the first 2 years of life. Since recent measles epi-demics in Korea may increase the late risk of SSPE, the authors investigated the clinical characteristics of SSPE focusing on brain MRI. Methods : Six cases(4 males, 2 females) of SSPE patients were retrospectively re-viewed for clinical, EEG, laboratory and brain MRI findings. Results : Four of 6 had a history of measles infection in the first year of life. Clinical manifestations were as follows : myoclonus(6), falling(4), ataxia(4), dysarthria(3), seizures(2), involuntary movements(2), tremor(2), head drop(1), sleep disturbance(1). In all cases, CSF IgG, CSF IgG/albumin ratio, and CSF/serum IgG index increased, oligoclonal bands were positive, and CSF antimeasles antibodies were positve. Frontal high amplitude δ activities and anteriorly-accentuated multifocal epileptiform discharges were noted on EEG. Brain MIRI revealed T2-weighted high signal intensity of the deep white matter. Conclusion : The diagnosis of SSPE depends on characteristic clinical features and elevation of measles antibodies in CSF, supported by others including EEG, CSF and brain MRI findings. We hope the clinical characteristics we mentioned may be useful for the early diagnosis and active management of SSPE in Korea.
