정신장애인의 재입원에 영향을 미치는 사회통제요인에 관한 탐색적 연구: 재발 개념에 대한 푸코 관점의 비판을 중심으로

국문 초록

본 연구는 푸코의 사회통제라는 관점에서 정신장애인의 재입원에 영향을 미치는 요인에 관하여 탐색하는 것이다. 연구대상은 전국정신장애인실태조사에서 면접조사를 통해 수집된 성인정신장애인 600명의 자료이여, 통계방법은 로지스틱회귀분석을 사용하였다. 분석결과 정신장애인의 재입원 여부에 영향을 미치는 사회통제요인으로는 장애등록여부(B=-.486, p<.05), 교육수준(B=.206, p<.05), 가구원수(B=-.309, p<.01), 외래빈도(B=-.251, p<.10) 등이 통계적으로 유의미한 영향을 미치는 것으로 나타났다. 이와 같은 연구결과의 함의를 정리해보면 다음과 같다. 첫째, 정신장애인의 재발 혹은 재입원은 사회통제의 관점에서 해석될 수 있다. 일상적 권력의 훈육에 순응할 가능성이 높은 고연령층과 정신장애 등록자일수록 재입원경험이 감소하며 교육수준이 높은 사람은 재입원경험이 증가하는 것으로 나타났다. 또한 일상적 훈육체계인 가구원의 수가 많아지거나 외래 정신의료서비스와의 접촉빈도가 증가할수록 재입원경험은 감소한다. 둘째, 불명확하고 재입원에 의존하는 재발개념은 정신장애인에 대한 지식적 통제도구로 기능하고 있어 재검토가 필요하다. 셋째, 재발은 재입원의 필요가 아니라 더 많은 돌봄의 요구로 재해석될 필요가 있다. 이러한 개념적 전환을 통해 재발상황은 당사자가 타자의 통제에 순응하는 방식이 아니라 본인의 원하는 방식으로 더 많은 돌봄을 제공받을 수 있도록 지원하는 지적 도구로 변화해야 한다. 넷째, 사회통제의 도구로서 기능하는 재발개념을 새로운 언어와 개념으로 대체해야할 필요가 있다.

영문 초록

This study is to explore the factors affecting the readmission of mentally disabilities persons from the perspective of Foucault s social control. The subjects of this study were data of 600 adults with mental disorders collected through interviews in the National Survey of Mental Disabilities, and logistic regression analysis was used as a statistical method. As a result of the analysis, the social control factors that influence the readmission of mentally disabled persons include disability registration (B=-.486, p<.05), and education level (B= .206, p<.05), number of household members (B=-.309, p<.01), outpatient frequency (B=-.251, p<. 10), etc. were found to have a statistically significant effect on the re-hospitalization experience. The implications of such research results are as follows. First, the relapse or readmission of a person with a mental disabilities can be interpreted in terms of social control. It was found that the readmission experience decreased for the elderly and those with mental disorders, and the readmission experience for those with a high level of education increased. As the number of household members increases or the frequency of contact with outpatient psychiatric services increases, the experience of readmission decreases. Readmissions decrease in groups that are likely to comply with the discipline of ordinary power, and readmissions decrease if there are many discipliners or contact frequency. Second, the concept of relapse, which is unclear and dependent on readmission, functions as a knowledge tool to control the mentally disabilities and needs to be reviewed. Third, relapse needs to be reinterpreted as a demand for more care, not a force for readmission. Through this conceptual shift, the relapse situation should be transformed into an intellectual tool that provides more care in the way the person wants, rather than the way the person adapts to the control of others. Fourth, it is necessary to replace the concept of relapse functioning as a tool for social control with a new language and concept.
