말 언어 문해기초 선별 체크리스트 개발과 선별 정확도 및 유용성 검증

국문 초록

배경 및 목적: 본 연구에서는 초등학교 입학 전 만 5-6세 아동을 대상으로 언어, 말, 문해기초 영역상의 문제를 선별할 수 있는 ‘말 언어문해기초 선별 체크리스트’를 개발하였다. 개발한 체크리스트가 선별검사 도구로서 정확하고 유용한지 검증하고자 신뢰도, 타당도, 민감도, 특이도, 양성예측도, 음성예측도를 확인하였다. 방법: 1차 검사로 선별 체크리스트에 참여한 아동 2,880명 중 2차 확인검사를 희망한 아동 155명을 대상으로 결과를 확인하였다. 확인검사 결과 언어, 말, 문해기초 영역에서 어려움을 보인 아동 110명은 고위험군 집단으로, 모든 영역에서 적절한 발달을 보인 아동은 일반 집단으로 분류하였다. 결과: 전체 문항에 대한 Cronbach’s α 값은 .884로 확인되었고, 언어발달 전문가 6인이 평정한 내용타당도 지수는 36개 문항 모두 .70 이상, 모든 문항의 내용 타당도 평균은 .91로 확인되었다. 2차 확인검사로 분류된 고위험군과 일반 집단의 선별 총점 차이를 통해 임상적 타당도도 확인하였다. 선별 체크리스트의 ROC 곡선 결과, AUC 영역 .858, 민감도 84.4%, 특이도 76.4%로 나타났다. 산출된 Cut-off 점수로 집단 분류된 데이터를 확인한 결과 양성예측도(PPV)는 92.3%로 확인되었다. 논의 및 결론: 학교 입학 전 언어발달지연 고위험군 아동을 선별하기 위해 개발한 ‘말 언어 문해기초 선별 체크리스트’는 신뢰도, 타당도, 분류 정확도가 적절한 것으로 나타났다. 본 체크리스트가 만 5-6세 언어발달지연 고위험군 아동을 선별하여 학교 생활의 실패를 예방하는 데 기여할 수 있길 바란다.

영문 초록

Objectives: This study aimed to develop the “Speech Language Early Literacy Screening Checklist” in order to identify speech, language, and early literacy difficulties in children aged 5-6 years before entering elementary school. This study examined the reliability, validity, sensitivity, specificity, positive predictive value, and negative predictive value to verify the accuracy and efficiency of the checklist as a screening tool. Methods: Out of the 2,880 children who participated in the initial screening test, 155 underwent diagnostic deep tests. A total of 110 children who exhibited difficulties in language, speech, and early literacy areas were classified as the high-risk group, while those who demonstrated appropriate development in all areas were classified as the general group. Results: The Cronbach’s α value for all checklist items was .884. The content validity index rated by six language development experts was 0.70 or higher for all 36 items, with an average content validity score of 0.91. Clinical validity was confirmed through the total score differences be tween the general and high-risk groups classified by the diagnostic deep tests. The receiver operating characteristic (ROC) curve results for the screening checklist showed an area under the curve (AUC) of 0.858, sensitivity of 84.4%, and specificity of 76.4%. The positive predictive value (PPV) calculated from the cut-off score was 92.3%. Conclusion: The “Speech Language Early Literacy Screening Checklist” developed to identify children at high risk of language development delays before school entry has shown appropriate reliability, validity, and classification accuracy. This study suggests that the implementation of this check list will contribute to preventing school failure by effectively identifying at-risk children aged 5-6.
