시뮬레이션 기반 인수인계 교육이 간호대학생의 지식, 비판적 사고 성향, 임상 판단력에 미치는 효과

국문 초록

목적 임상 간호사는 병동 업무의 특수성으로 인해 3교대 근무를 하게 되며 매 근무조별로 간호업무 인수인계가 있다. 간호사의 인수인계는 간호의 연속성을 유지하여 환자의 안전과 간호 및 의료의 질을 보장한다. 인수인계는 환자 정보를 교환하는 중요한 간호 활동이며, 이 정보를 정확하게 전달하는 것은 의료진의 책임과 의무이다. 더불어 시뮬레이션기반 교육은 간호학 및 의학에서 환자에게 유해한 결과를 초래하지 않고 임상 수행을 할 수 있도록 기회를 제공하는 교육 방법이다. 본 연구는 간호대학생을 대상으로 시뮬레이션 기반 인수인계 교육 프로그램을 개발하고 적용하여 인수인계 지식, 비판적 사고 성향, 임상 판단력을 평가하기 위해 시행하였다. 방법 본 연구는 시뮬레이션 기반 인수인계 교육 프로그램을 개발하고 운영하여 효과를 검증하기 위한 단일군 전후실험 설계이다. 경기도 소재 일 간호대학 간호학과에 재학 중인 4학년 학생을 대상으로 2018년 2월 19일부터 3월 2일까지 자료 수집하였다. 자료 분석은 SPSS Version 22.0 프로그램을 활용하여 일반적 특성은 실수, 백분율 및 평균과 표준편차로 분석하였다. 교육 프로그램 참여 전과 후 대상자의 인수인계 지식, 비판적 사고 성향, 임상 판단력은 paired t-test를 이용하여 분석하였다. 결과 인수인계 지식은 교육 전 평균은 4.63±1.61에서 교육 후 5.83±0.95로 유의한 차이가 있었다(t=-3.71, p=0.001). 비판적 사고 성향은 교육 전 평균 3.39±0.46에서 교육 후 3.75±0.42로 유의한 차이가 있었다(t=-4.69, p<0.001). 임상 판단력은 교육 전 평균 1.79±0.37에서 교육 후 3.15±0.48로 유의한 차이가 있었다(t=-15.67, p<0.001). 결론 본 연구의 결과 시뮬레이션 기반 인수인계 교육 프로그램은 간호대학생의 인수인계 지식, 비판적 사고 성향, 임상 판단력 향상에 효과적임을 확인하였다. 이 세 가지 요인은 간호사의 인수인계시 필요한 기본 역량이며, 간호대학생을 대상으로 이를 활용한 인수인계 교육을 실시하고 효과를 확인하였다는데 의의가 있다. 따라서, 본 연구의 결과를 토대로 향후 인수인계 수행능력 향상을 위한 교육 체계 정립의 기초 자료로 활용될 것으로 기대하며, 신규간호사 및 유휴 간호사를 위한 교육에도 활용되어 향후 인수인계의 질과 환자안전문화 향상에 기여할 수 있을 것이라고 사료된다.

영문 초록

Purpose Clinical nurses work in three shifts due to the specificity of the ward's work, and there is a handoff of nursing work for each shift. The nurse's handoff maintains the continuity of nursing care, ensuring the safety of patients and the quality of nursing care and medical care. Handoff is an important nursing activity that exchanges patient information, and it is the responsibility and duty of medical staff to accurately deliver this information. In addition, simulation-based education is an educational method that provides opportunities for clinical practice without having harmful results in nursing and medicine. This study was conducted to evaluate handoff knowledge, critical thinking disposition, and clinical judgment by developing and applying a simulation-based handoff education program for nursing college students. Method This study is a one-group pretest-posttest design to develop and operate a simulation-based handoff training program to verify its effectiveness. The subjects were 4th year students enrolled in the Department of Nursing at the College of Nursing in Gyeonggi-do, The data were collected from February 19 to March 2, 2018. For data analysis, SPSS Version 22.0 was used, and general characteristics were analyzed by numbers, percentages, averages, and standard deviations. Before and after participating in the educational program, the subject's handoff knowledge, critical thinking disposition, and clinical judgment were analyzed using a paired t-test. Results Thirty people participated in this study as subjects. All participants were female and the average age was 22 years. The main results of the study are as follows. There was a significant difference in handoff knowledge from 4.63±1.61 before education to 5.83±0.95 after education (t=-3.71, p=0.001). There was a significant difference from 3.39±0.46 before education to 3.75±0.42 after education (t=-4.69, p<0.001). There was a significant difference in clinical judgment from 1.79±0.37 before education to 3.15±0.48 after education (t=-15.67 , p<0.001). Conclusion As a result of this study, it was confirmed that the simulation-based handoff education program was effective in improving the handoff knowledge, critical thinking disposition, and clinical judgment of nursing students. Therefore, based on the results of this study, it is expected that it will be used as basic data for establishing an education system to improve the performance of handoff in the future. In addition, it is considered that it can be used for education for new nurses and idle nurses, contributing to the improvement of the quality of handoff in the future.
