GLMM을 이용한 지역간 건강도 비교 연구

국문 초록

본 연구에서는 지역사회건강조사에서 얻은 자료를 분석하여 지역별 건강 수준을 측정하는데사용되는 직접 표준화 방법의 문제점을 파악하고, 일반화 선형혼합모형(GLMM: Generalized Linear Mixed Model)에 기반한 새로운 표준화 방법을 제시하였다. 제안한 표준화 방법이 기존 직접 표준화 방법의 대안으로의 가능성을 평가하기 위해 2019년 지역사회건강조사 데이터를 사용하였다. 이 자료를 사용하여, 새로운 표준화 방법, 2005년 전국 인구수를 기반으로 한 직접 표준화 방법, 2019년 전국 인구수를 기반으로 한 직접 표준화 방법을 통해 세 가지의 지역별 건강수준 표준값을 구하여 비교분석하였다. 이를 통한 연구 결과를 요약하면 다음과 같다. 첫째, 현재 사용하고 있는 직접 표준화 방법은 2005년 추정된 인구를 기준으로 하고 있으나 고령화가 많이 진행되고 있는 현 인구구조를 반영하지 못하고 있다. 둘째, GLMM 기반 표준값은 2005년 표준화율보다 2019년 표준화율과 더 높은 상관관계를 나타내어 제안된 표준화 방법이 인구구조 변화를 충분히 반영하고 있음을 의미한다. 마지막으로, 직접 표준화율과 GLMM 기반 표준값의 변수 간의 상관분석 결과 GLMM 기반 표준값이 직접 표준화율에서 나타나는 질병 변수와 건강 관련 변수의 관계를 대부분을 가지고 있었다.

영문 초록

In this paper, we first examined the problem of the direct standardization method, which is mainly used to measure the regional health level by analyzing the data obtained from the Korea Community Health Survey, and proposed a new standardization method based on the Generalized Linear Mixed Model (GLMM). In order to evaluate the performance of the proposed standardization as an alternative to the existing direct standardization method, we used the data which obtained from the Korea Community Health Survey conducted in 2019. Using this data, we compared and analyzed three regional health level standard values through the new standardization method, GLMM, the direct standardization method based on the national population in 2005, and the direct standardization method based on the national population in 2019. The results of this study can be summarized as follows. First, the direct standardization method currently used is based on the national population estimated in 2005, but it does not reflect the current demographic structure that is aging much. Second, the GLMM-based standard value correlated higher with 2019 standardization rate than with 2005 standardization rate, which implies that the proposed standardization method sufficiently reflects demographic changes. Finally, GLMM-based standard value was related to the disease variables and health-related variables, which were mostly similar to those shown in the direct standardization rates.
