외래 이용자의 의료서비스 안전관리에 대한 인식 및 관련 요인

국문 초록

긍정적인 의료서비스를 경험한 환자들은 증상 완화 및 치료에 대한 순응도가 높게 나타나 환자가 중심이 되는 의료문화 형성 및 보건의료서비스 개선의 필요성이 높아지고 있다. 이를 위해 본 연구는 외래 의료서비스 이용 시 환자안전관리 경험을 분석하고, 의료서비스 만족도에 미치는 영향을 규명하여, 효과적인 의료서비스 안전관리 구축을 위한 기초 자료를 제공하고자 수행하였다. 2020년 의료서비스경험조사의 원자료(Raw data)를 MDIS(MicroData Integrated Service)에서 제공받아 시행한 2차 분석 연구로, 연구 대상자는 15세 이상 의원 및 병원의 외래 이용자 6,883명으로 수집된 자료는 SPSS window 26.0 통계 프로그램을 이용하여 분석하였다. 분석방법은 첫째, 인구사회학적 및 건강상태, 의료기관 유형 특성은 빈도분석, 해당 특성별로 의료서비스 안전관리 항목별 차이 검증은 기술통계, 의료서비스의 만족도에 영향을 미치는 요인을 분석하기 위해 다중회귀분석을 실시하였다. 연구결과 첫째, 외래 환자안전관리 항목 중 진료 전 신분확인 안전성이 가장 높게 확인되었으나, 교육수준을 고려한 수진자 홍보와 의료기관의 책임성 수립이 지속적으로 필요하다. 주사투여 이유에 대한 설명을 듣지 못하는 사람이 15-19세의 청소년, 낮은 교육수준, 의료급여, 저소득층, 만성질환자 등의 취약계층에서 높게 나타나 의료기관에서 이들에 대한 주의가 필요하다. 투약부작용 경험이 있어, 복용 약물의 적절성을 통합적으로 검토하고 부작용 발생 여부를 파악, 교육할 필요가 있다. 또한 외래 이용 시 감염불안을 느끼고 있어, 감염 예방의무 등을 철저히 이행해야 하고, 조직적이고 효과적인 의료기관 감염관리체계를 수립하여야 할 것이다. 주사제 투여시의 감염 관리인 주사 전 의료진 손소독, 밀봉된 주사제 및 주사 의료용품 개봉, 주사 전 피부 소독을 꾸준히 실천하여 환자의 감염 불안을 해소해야 할 것이다. 둘째, 의료기관의 의료서비스 전반적 만족도에 의사 및 간호사 만족도, 의료기관 만족도와 함께 8개 안전관리 위험도가 영향을 미치고 있었으므로, 안전관리가 의료서비스의 기본임을 확인하고, 이 안전관리를 필수적으로 수행함으로써, 이용자들의 의료서비스 만족도를 높여야 할 것이다.

영문 초록

Patients who experienced positive medical services showed high adherence to symptom relief and treatment, increasing the need to form a patient-centered medical culture and improve health care services. To this end, This study was conducted to provide basic data for the establishment of effective medical service safety management by investigating the effect of patient safety management experience and medical service satisfaction when using outpatient medical services. This is a secondary analysis study conducted by receiving raw data from the 2020 Medical Service Experience Survey from MDIS(MicroData Integrated Service), with 6,883 outpatients aged 15 or older and hospitals, and the collected data were analyzed using the SPSS window 26.0 statistical program. The analysis method is first, demographic and health status, frequency analysis for characteristics of medical institution type, descriptive statistics for each characteristic and verification of differences in medical service safety management items for each characteristic, and multiple regression analysis to analyze factors affecting satisfaction with medical services was carried out. As a result of the study, first, the safety of pre-treatment identification was confirmed to be the highest among outpatient safety management items, but it is necessary to continuously promote the recipient considering the level of education and establish responsibility of medical institutions. The number of people who cannot hear the explanation of the reason for injection is high in vulnerable groups such as teenagers aged 15-19 years, low education levels, medical benefits, low-income families, and chronically ill patients, so medical institutions need to pay attention to them. Since medication side effects exist, it is necessary to comprehensively review the appropriateness of the medication, identify and educate who on the occurrence of side effects. Since infection anxiety is felt during outpatient use, the duty to prevent infection should be thoroughly fulfilled, and an organized and effective infection control system should be established at medical institutions. When administering injections, it is necessary to steadily practice hand disinfection of medical staff before injection, opening sealed injections and injection medical supplies, and sterilizing skin before injection to relieve infection anxiety in patients. Second, since the overall satisfaction of medical services by medical institutions, along with the satisfaction of doctors, nurses, and medical institutions, eight safety management risks were affecting them, confirming that safety management is the basis of medical services.
