국문 초록

본 연구는 지역사회에 거주하는 당뇨병 환자를 대상으로 보건소 방문건강관리사업을 통한 8주간의 당뇨집중건강관리 프로그램을 적용하여 당뇨증상경험, 당뇨에 대한 지식, 자가관리 자신감, 자가관리 이행수준과 혈당에 미치는 효과를 파악하기 위해 시도되었다. 본 연구는 비동등성 대조군 전ㆍ후설계를 이용하였으며, 연구대상자는 실험군 30명, 대조군 30명으로 총 60명이다. 집중건강관리 프로그램은 방문간호사에 의해 8주간 운영되었으며, 자료수집기간은 2013년 8월부터 11월까지였다. 연구결과 집중건강관리 프로그램이 대상자의 당뇨에 대한 지식(z=-3.184, p=.001), 자가관리 자신감(z=-3.352, p=.001), 자가관리 이행수준(t=4.520, p<.001)을 향상시켰으며, 식후 2시간 혈당(z=-2.587, p=.010), 당뇨관련 증상(z=-2.376, p=.018)을 감소시켜 당뇨관리에 매우 효과적인 접근법이라 할 수 있겠다. 따라서 지역사회의 건강형평성 제고를 위해 집중건강관리사업의 확대를 위한 예산과 인력지원이 필요하며, 집중건강관리 프로그램 종료 후 당뇨교육의 효과를 지속시키기 위해 계속적인 추후관리 프로그램이 필요하다고 본다.

영문 초록

The purpose of this study was to investigate the effects of intensive health management program for diabetic patients in home visiting health service of public health center on diabetic symptoms, knowledge, confidence for self-care, self care behaviors and glycemic level. This study was designed with nonequivalent control group pretest-posttest design. Participants included 60 diabetic patients were assigned to an experimental group (30) or control group (30). The program was conducted for 8 weeks by home visiting nurses. Data were collected from August to November, 2013. The results showed that intensive health management program was effective in improving knowledge (z=-3.184, p=.001), confidence for self care (z=-3.352, p=.001), self care behaviors (t=4.520, p<.001). Glycemic level (z=-2.587, p=.010) and diabetic symptoms (z=-2.376, p=.018) were decreased in experimental group. These results indicate that the intensive health management program may be an effective approach to the management for diabetic patients in the community.
