약물의존자들에서 MMPI-2의 MAC-R, AAS 및 APS의 진단적 분류 정확성

국문 초록

약물의존은 성격적 및 임상적 특성이 매우 다양하고 이질적이어서 치료 개입과 결과, 재발예방을 위한 정확한 진단과 평가는 매우 중요하다. 본 연구는 ROC 분석을 활용하여 152명의 남성정신과 환자군과 280명의 약물의존군(메스암페타민과 흡입제의존군) 총 432명을 대상으로 MMPI-2 보충척도에 포함된 세 가지 물질관련 척도들(AAS, APS와 MAC-R)의 진단적 분류 정확성을 비교해 보았고, 이 세 척도들 각각의 최적 절단점들을 산출하였다. AAS가 가장 민감하였던 선행연구 결과들과는 달리, 우리나라 약물의존자들의 경우 약물의존 진단에 있어서 APS가 AAS나 MAC-R보다도 더 정확하였다. AAS, APS와 MAC-R의 최적 절단점들을 산출하였는데 약물의 존군과 정신과 환자군 간의 APS의 최적 절단점은 T≥58점, AAS는 T≥52점, MAC-R은 원점수≥20점(T≥47)으로 나타났다. 이와 같이 본 연구의 결과들은 약물의존자들의 효과적인 치료적 개입을 위해 정확한 진단과 평가의 필요성과 개입에 있어서 다양한 심리적, 성격적 및 임상적 특성들을 고려해야 함을 시사해 준다. 이와 더불어 임상가들이 치료 장면에서 약물의존자를 만나게 될 때, 본 연구의 결과를 토대로 MMPI-2와 물질관련 보충척도들의 정보를 적용한다면 그들에 대한 평가적 혹은 치료적 정보를 더욱 정확하고 풍부하게 활용할 수 있을 것으로 기대한다.

영문 초록

Drug dependence indicates various idiosyncratic personality and clinical features. The precise diagnostic classification is crucial for treatment intervention and outcome, and relapse prevention. This study examined diagnostic efficiency and clinical application of the MMPI-2 substance abuse scales, such as MAC-R, AAS and APS by comparing drug dependents with psychiatric inpatients. A total of 432 male participants including 152 psychiatric inpatients and 280 illegal drug dependents (138 methamphetamine dependents, 142 inhalants dependents) had a semi-structured psychiatric interview and filled out the MMPI-2. Unlike the previous research findings, the APS was found the most accurate measurement scale for drug dependents with a high diagnostic accuracy and predictive validity. The diagnostic optimal cutoff points of the APS, AAS, and MAC-R for drug dependents were found as T-score≥58, T-score≥52 and raw score≥20 respectively. This study identified the APS scale of the MMPI-2 as the most accurate to differentiate drug dependents. Since accurate diagnostic classification as well as identifying psychological and clinical features should be taken into account for the treatment and intervention, the result is expected to contribute for illegal drug dependents as well as clinicians in clinical settings.
