노인의 거주유형별 일상생활 수행능력의 장애, 지각된 건강상태 및 우울의 비교연구

국문 초록

본 연구는 노인의 거주유형에 따른 특성을 분석하고, 일상생활 수행능력의 장애, 지각된 건강상태 및 우울정도를 비교분석함으로써 시설노인과 재가노인의 건강을 유지, 증진하여 삶의 질을 향상시키기 위한 개별적이고 체계적인 간호중재방안을 모색하기 위한 근거자료를 제공하고자 시도하였다. 본 연구의 대상자는 B광역시에 거주하는 60세 이상의 재가노인 223명과 시설에 입소한 200명이었으며 자료의 수집은 2008년 6월 1일부터 2008년 8월 20일에 걸쳐 연구도구를 사용하여 일대일면담에 의해 일상생활 수행능력의 장애, 지각된 건강상태 및 우울을 조사하였다. 자료는 SPSS WIN 14.0을 이용하여 t-test, one-way ANOVA, χ2-test, Fisher exact test 및 Pearson correlation coefficient를 이용하여 분석하였다. 연구결과 재가노인과 시설노인은 일상생활 수행능력의 장애, 지각된 건강상태 및 우울에서 통계적으로 유의한 차이가 있었다. 시설노인에서 일상생활 수행능력의 장애와 지각된 건강상태와 일상생활 수행능력의 장애와 우울 간에 유의한 역상관관계가 있었고 지각된 건강상태와 우울 간에 유의한 역상관관계가 있었다. 재가노인에서는 일상생활 수행능력의 장애와 우울 간에 유의한 순상관관계가 있었고 지각된 건강상태와 우울 간에 유의한 역상관관계가 있었다. 따라서 거주유형별 노인의 특성에 알맞은 일상생활 수행능력 장애를 지연시키고 지각된 건강상태를 향상시킬 뿐 만 아니라 우울을 감소시키기 위한 간호중재 프로그램이 필요하다.

영문 초록

The purpose of this study was to compare and the differences between the elderly in their home and in institution in terms of their activity of daily living(ADL), perceived health status and depression. A convenienced random sampling method was conducted to recruit participants from June 1 to August 20 of 2008. Activity of daily living were measured by using Index of ADL, depression by using Geriatric depression scale short form Korea(GDSSF-K) in 423 elderly living home and institution. There were significant differences in ADL, perceived health status and depression between the elderly in home and in institution. There was a significant relationship between perceived health status and depression, and there was a significant relationship between perceived health status and ADL in elderly living institution. However, there was no significant relationship among the variables in elderly living home except relationship between depression and perceived health status. The finding of this study gives useful information for constructing an intervention program and care for elderly according to types of residency.
