국내 C형간염 발생 위험지역 탐색

국문 초록

C형간염은 바이러스가 간에 혈액잔피가 발생하며 20~30년 후 간경변이나 간암으로 발전되는 질환이다. 주로 40대 이상에서 발병률이 높고 남성보다는 여성의 발병률이 더 높다고 보고되고 있다. C형간염 발생자수는 매년 지속적으로 증가하고 있어 사회·경제적 부담이 점점 커지고 있다. 지금까지 C형간염의 감염경로, 발병률, 간 질환으로의 발전 등에 관한 연구가 대부분이며 C형간염의 고위험 발생지역 탐색에 대한 연구는 미흡한 실정이다. 본 연구의 목적은 2004년 이후 14년간 발생한 국내 C형간염 발생 자료를 스캔통계학의 포아송 모형과 시공간 순열 모형으로 분석하여 고위험 클러스터와 실린더를 탐색하는 것이다. 최대우도 로그우도비와 Monte Carlo 가설검정을 이용하여 국내 C형간염의 발생 위험 지역을 탐색한 결과, 포아송 모형으로는 부산 서구, 서울 광진구, 시공간 순열 모형으로는 부산 서구와 충남 아산시가 가장 위험한 클러스터의 중심이라는 결론을 얻었다. 본 연구 결과에 근거한 국내 C형간염의 고위험 지역과 발생시기에 관한 정보는 C형간염의 예방 표본 감시체계에 유용하게 활용될 수 있을 것으로 예상한다.

영문 초록

Hepatitis C is a virus that infects the liver and is a blood-borne disease. It is reported that the incidence is high in the age group of 40 years or older and the incidence of the female is higher than that of the male. In recent years more patients of Hepatitis C are being reported by KCDC (Korean Center for Disease Control and Prevention). Even though the burden of socioeconomic burden of Hepatitis C is getting bigger, the statistical study on the occurrence rate and the high risk clusters is not of interest and is in the early stage. The purpose of this study is to investigate the high risk clusters of Hepatitis C infection in Korea for 14 years after 2004. Using the maximum likelihood ratio and the Monte Carlo hypothesis test, we found 25 clusters and 9 cylinders that most of them are the regions of high-density in population or coastal area. Based on the results of this study, the information on the high-risk cluster of Hepatitis C in Korea is expected to be useful for the prevention surveillance of Hepatitis C.
