해면정맥동염에 의한 완전 동안신경마비를 동반한 안부대상포진 1예

국문 초록

목적: 안부대상포진 환자에서 해면정맥동염에 의하여 동공침범을 동반한 우측 완전동안신경마비 1예를 경험하였고, 항바이러스 제제및 스테로이드 복용 12일만에 빠른 완치를 획득하였기에 이를 보고하고자 한다. 증례요약: 60세 남자 환자가 3일 전부터 시작된 우측 눈꺼풀의 부종 및 수포를 주소로 내원하였다. 환자는 1주 전부터 근육통, 기침,열감, 두통이 있어 근처 병원에서 대증적 치료를 받았었다. 환자는 피부과에서 경구 항바이러스 제제(famciclovir 250 mg) 하루 2회및 스테로이드 제제(methylprednisolon 4 mg) 하루 1회를 1주일간 복용하고 중단하였다. 이후 부종 및 수포는 호전되는 양상을 보였으나 우안의 눈꺼풀처짐, 안구운동장애(상전, 하전, 내전) 및 우안 동공 산대가 관찰되었고 복시를 호소하였다. 뇌자기공명영상에서우측 해면정맥동에 조영증가되는 염증성 병변을 보여 해면정맥동염에 의한 동공침범을 동반한 우측의 완전 동안신경마비로 진단하고 항바이러스 제제(famciclovir 250 mg) 하루 3회, 경구 스테로이드(prednisolone 40 mg) 하루 1회를 처방하였다. 항바이러스 제제 및스테로이드 사용 다음 날부터 눈꺼풀처짐 및 안구운동장애가 좋아지기 시작하였으며 12일 후에는 완전히 호전되었다. 결론: 안부대상포진 환자에서 포도막염이나 뇌혈관류 없이도 동공을 침범한 안구운동장애를 동반할 수 있으며, 항바이러스 제제 및스테로이드를 적극적으로 사용하는 것이 증상의 빠른 호전에 도움이 될 것으로 생각된다.

영문 초록

Purpose: To report a case of complete oculomotor nerve palsy with pupil involvement complicated by inflammation of the cavernous sinus. Complete resolution was obtained after 12 days of antiviral and steroid treatments. Case summary: A 60-year-old male presented with edema and vesicles of the right upper eyelid. The patient had myalgia,cough, fever and headache 1 week earlier and was treated with conservative therapy. The patient received an antiviral agent(famciclovir 250 mg) twice a day and steroid agent (methylprednisolon 4 mg) once a day at the dermatology department for 1week. The eyelid edema and vesicles improved. However, ptosis, ocular movement limitation, mydriasis of the right eye and diplopia occurred. Brain magnetic resonance imaging revealed hyperintensity in the right cavernous sinus with enhancement, implicating inflammation. The patient was diagnosed with right complete oculomotor nerve palsy with pupil involvement. An antiviral agent (famciclovir 250 mg) three times a day and a steroid agent (prednisolone 40 mg) once a day were prescribed. From the next day, ptosis and ocular movement limitation improved and 12 days later, completely resolved. Conclusions: Ocular movement limitation and mydriasis can be accompanied by herpes zoster ophthalmicus without uveitis and cerebral aneurysm. Administering active antiviral and steroid treatment to obtain rapid resolution is important.
