원추각막 콘택트렌즈의 장기적 효과 및 안전성

국문 초록

목적: 원추각막 환자에서 YK-KC 렌즈Ⓡ (LucidKorea Ltd., Seoul, Korea)의 장기 효과 및 안전성을 보고하고자 한다. 대상과 방법: 원추각막을 진단 받고 YK-KC 렌즈Ⓡ를 5년 이상 착용해 온 97명, 152안을 대상으로 렌즈 착용감에 대한 만족도를 조사하였고, 렌즈처방 전과 최종경과관찰 시의 최대교정시력을 비교하였으며, 각막곡률값을 이용한 원추각막 진행 여부 및 각막 합병증을 정량적으로 평가하였다. 결과: 총 97명 중 남자 57명, 여자 40명이었고, 평균 나이는 28.6 ± 8.5세였으며, 평균 경과관찰 기간은 8.0 ± 2.9년이었다. 렌즈착용감에 대한 설문조사에서 126안(82.9%)에서 편안하다고 답하였고, 렌즈착용 전 최대교정시력(logMAR)은 0.49 ± 0.42에서 렌즈착용 후 최종경과관찰 시 0.19 ± 0.27로 유의하게 호전되었다(p＜0.001). 최대 및 최소 Simulated keratometry (Sim K) 값은 렌즈착용전 각각 48.84 ± 7.20 D, 48.60 ± 6.26 D에서 최종경과관찰 시 각각 49.24 ± 6.26 D, 48.90 ± 7.27 D로 증가하였으나 유의한차이는 보이지 않았고(각각 p=0.66, p=0.11), 각막난시는 4.12 ± 4.38 D에서 3.60 ± 2.78 D로 감소하였으나 유의한 차이는 보이지않았다(p=0.22). 경과관찰 중 24안(16%)에서 점상 혹은 융합 상피결손이 관찰되었으나, 지속적 전층상피결손은 발견되지 않았다. 결론: YK-KC 렌즈Ⓡ는 시력교정이 어려웠던 원추각막 환자에서 우수한 착용감과 함께 시력 향상 효과를 얻을 수 있었고, 장기간 사용시에도 원추각막의 진행 및 심각한 각막합병증을 유발하지 않아 효과적이고 안전한 치료 방법으로 생각된다.

영문 초록

Purpose: To evaluate the long-term clinical effect and safety of YK-KC lens？？ (LucidKorea Ltd., Seoul, Korea) for keratoconus. Methods: In this study we investigated 152 keratoconic eyes fitted with YK-KC lens？？ and followed up for at least 5 years. We assessed retrospectively self-reported patient comfort, best corrected visual acuity, corneal topographic indices before and after contact lens fitting and complications during contact lens wearing. Results: The study included 57 male and 40 female patients with a mean age of 28.6 ± 8.5 years. The mean follow-up was 8.0± 2.9 years. Regarding lens comfort, 126 eyes (82.9%) showed self-reported comfort for YK-KC lenses？？. The mean best corrected visual acuity (log MAR) improved from 0.49 ± 0.42 before lens fitting to 0.19 ± 0.27 after lens wearing, which was statistically significant (p < 0.001). Based on the keratometric values, after contact lens fitting both Simulated keratometry (Sim K) max and Sim K min tended to be steeper, but these differences were not statistically significant (p = 0.66 and 0.11, respectively). There were no statistically significant differences between the values before and after fitting with respect to the astigmatic powers(p = 0.22). Complications observed included punctate or coalesced epithelial corneal staining in 24 eyes (16%), however,persistent full-thickness epithelial defect was not observed. Conclusions: The YK-KC lens？？ in patients with keratoconus can provide excellent visual improvement and comfort without any significant influence on the progression of keratoconus or corneal complications during a long-term follow-up period.
