원추각막 및 각막확장증에서 각막링 삽입술의 임상성적 비교분석

국문 초록

목적: 원추각막 및 각막확장증에서 인택스링을 이용한 각막링삽입술의 임상성적을 각막이식의 임상성적과 비교 분석하고자 하였다. 대상과 방법: 원추각막 및 각막확장증에서 2011년부터 2013년까지 인택스링을 삽입받고 3개월 이상 추적 관찰된 16명의 의무기록을 후향 분석하였다. 같은 질환으로 각막이식을 받고 1년 이상 추적 관찰된 30명과 비교하여 치료 후 시력(logMAR), 각막지형도의 변화,합병증을 비교 분석하였다. 링 삽입군에서 술 후 전안부 빛 간섭 단층촬영으로 측정한 삽입 깊이에 따른 교정 효과의 차이도 비교분석하였다. 결과: 수술 1년 뒤 링 군에서 logMAR 교정시력은 0.32로 0.20인 이식군에 비하여 낮게 나왔고, 시력 개선 정도 역시 링 군이 -0.23,이식군이 -1.51로 링 군이 교정효과가 작았다. 링 군의 술 후 각막곡률은 평균 45.20 ± 4.17D로 이식군에 비해 더 작은 곡률을 보였다. 3 mm 영역 부정난시는 6.93D로 이식군보다 컸다. 흔한 합병증은 링 주변 각막침착이 70%이었으나 시력에 영향은 없었다. 링삽입 깊이가 얕게 삽입된 군에서 각막곡률이 -8.49D 감소하여 깊게 삽입된 군보다 효과가 컸다. 결론: 원추각막 및 각막확장증에서 링 삽입술은 각막이식을 대치할 수는 없으나 교정효과가 있었다.

영문 초록

Purpose: To report the clinical results after the implantation of intrastromal corneal ring segments (Intacs？？) for the correction of keratoconus or keratectasia. Methods: This retrospective study was comprised of 16 eyes treated by insertion of intrastromal corneal ring and 30 eyes treated by penetrating keratoplasty (PKP) who were diagnosed with keratoconus or keratectasia. Visual acuity, refractive outcome, keratometric values were evaluated before and at 3 months, 6 months, and 12 months postoperatively. In addition, the implanted ring segment depth was measured by anterior segment optical coherence tomography and the results were compared based on the depth of the ring. Results: Twelve months after treatment, best corrected visual acuity (BCVA) was log MAR 0.32 at the ring group and log MAR 0.20 at the PKP group. BCVA change was larger at the PKP group than the ring group. Postoperative keratometric value was smaller at the ring group than at the PKP group. 3 mm irregular astigmatism was larger at the ring group than at the PKP group. The shallowly implanted ring group had a larger effect than the deeply implanted ring group. Conclusions: Intrastromal corneal ring segment implantation appears to be effective in improving the visual acuity and refractive outcome, although it cannot substitute for PKP.
