주간보호 이용노인 가족수발자의 서비스 만족도에 영향을 미치는 요인들

국문 초록

본 연구는 주간보호 서비스의 개선 방향을 파악하기 위해 현황을 요약하고 서비스 이용노인 가족수발자의 만족도 및 그에 영향을 미치는 요인들을 확인하고자 하였다. 서비스 만족도에 영향을 미치는 요인들을 검증하기 위해 96명의 주간보호 이용노인의 가족 수발자로부터 확보한 자료를 회귀분석하였으며 41개 주간보호센터로부터 수집된 자료는 현황파악을 위해 기술적 분석이 실시되었다. 분석결과를 요약하면 다음과 같다. 먼저 현황을 보면 사회복지사, 간호사/조무사를 제외한 전문인력을 제대로 확보하지 못했으며 치매, 중풍, 기타질환을 가진 노인들을 함께 대상으로 하여 서비스를 제공하는 센터가 많았고 정원초과, 중증 등의 이유로 서비스를 신청했는데도 이용하지 못하는 노인들이 있었다. 대부분의 센터에서 다양한 프로그램을 갖추고 있으며 서비스 제공에서 어려운 점으로는 프로그램 실시에 관련된 문제가 가장 많았고 그밖에 인력부족, 예산부족, 시설부족, 자원봉사자 문제, 공간부족 등의 순서로 지적되었다. 다음으로 주간보호 이용노인 가족 수발자의 서비스 만족도 및 그에 영향을 미치는 요인들에 대한 분석결과에 의하면 우선 주간보호에 대한 수발자들의 만족도는 매우 높다. 주간보호 이용노인 가족 수발자의 서비스 만족도에 영향을 미치는 요인들을 살펴보면 노인의 특징, 수발자의 특징, 서비스 관련요인, 서비스가 수발자에게 도움이 되었다는 주관적 인식에 관한 변수들 가운데 서비스가 실제로 도움이 되었다는 주관적 인식의 정도가 가장 중요한 결정 요인이며 그밖에 간병자의 정신적 건강 문제, 서비스 이용시간, 서비스에 관한 정보부족 문제의 경험여부가 유의미한 영향력을 갖는 것으로 나타났다. 이러한 연구결과에 따라 노인과 가족 수발자의 욕구를 반영하여 개별화된 서비스를 제공하고 이것이 실제로 도움이 되고 있는지 자주 점검함으로써 서비스에 의해 실제로 도움을 받고 있다는 인식을 향상시킬 것, 개별 상담과 집단 프로그램 등을 통해 간병자의 정신적 건강을 향상시킬 것, 주간보호센터의 확충을 통해 서비스 이용횟수와 시간을 늘려줄 것, 주간보호에 관한 정보를 충분히 제공할 것 등 실천적 함의가 제시되었다.

영문 초록

This study examined the factors affecting satisfaction among family caregivers of the older persons using adult day services. Before turning to it, description of some aspects in the current status of the services were also presented. The survey data collected from two sources - (1)41 social workers of the adult day care centers (2)96 family caregivers of the adult day service users - were used for analyses. The major results are as follows. First, descriptive analyses of the current status of the services show that more than half of the centers do not have medical consultants and rehabilitative therapists although most of them have social workers and nurses(aid). Some elders who applied for the services are not admitted because of the limited number, severity of the symptoms, lack of transportation, etc. Most of the centers have comprehensive programs including counseling, education, exercise, health care, medical evaluation, special diets, recreation. Service providers express difficulties in developing and operating programs, staffing, financing, facilities, management of volunteers, etc. Second, Family caregivers of the aged using adult day care showed high level of satisfaction in the services. Regression analyses of the variables affecting satisfaction of the family caregivers in adult day care show that their subjective perception of helpfulness of the services has the most powerful impact on it. The level of mental health problems of the caregivers as well as the lack of information about using the services have negative effects on their satisfaction while the frequency and hours of the service use have positive effects on it. According to the results, implications such as increasing the level of subjective perception of the helpfulness of the services among caregivers by frequent monitoring, improving the mental health of the caregivers by counselling and group programs, increasing the service hours, providing sufficient information about the adult day services were discussed.
