입원노인환자의 사례관리 프로그램 요구조사

국문 초록

본 연구는 입원 노인환자의 사례관리 요구도를 파악하여 병원에서의 노인사례관리 프로그램을 개발하기 위한 기초자료로 활용하기 위하여 실시되었다. 대학병원과 종합병원에 입원한 노인환자 중 퇴원예정이 있는 노인 290명을 대상으로 직접 설문을 실시하였다. 그 결과, 입원 노인환자의 1년간 재입원의 경험이 있는 노인이 조사대상의 71.4%였고 질환별로는 뇌졸증(29.0%), 소화기계 악성신생물(17.2%)순으로 많게 나타나 반복적 입원과 만성 질환의 특성을 보였다. 퇴원시의 일상생활 수행능력은 전체의 22.1％만이 모든 항목을 독립적으로 수행할 수 있다고 응답하였으며, 인지기능상태도 정상인 경우가65.2％정도로 나타나 퇴원 후 계속관리가 필요하였다. 정보적 간호요구도, 신체적 간호요구도, 정서적 간호요구도의 세가지 측면의 간호요구도는 모두 중간값보다 높게 나타나, 퇴원시 지속적인 간호를 필요로 하고 있었다. 사례관리 서비스 중 가장 요구도가 높은 서비스는 퇴원 후 지속적인 전화건강상담이었으며, 재가환자 관리를 위한 교육ㆍ상담교실, 가정간호 서비스, 전문인의 개별적인 상담서비스, 타의료기관 및 요양시설에 대한 정보제공이나 의뢰서비스의 순으로 원하고 있었다. 노인인구의 증가와, 만성질환 증가로 인하여 장기요양에 대한 요구가 증가하고 있으나, 이에 대한 대책은 아직 마련되어 있지 않은 상태이다. 병원은 의료의 질을 유지하면서 비용을 절감해야 하는 상황에 직면해 있다. 또한 본 연구결과 퇴원시 입원노인환자는 지속적인 서비스를 필요로 하고 있었다. 결국, 서비스 제공자와 대상자 모두에게 효율적인 방안으로서 입원노인환자를 위한 사례관리의 도입과 적용이 바람직할 것이며, 이를 위해 구체적이고 명확한 사례관리 프로그램이 지속적으로 연구되어야 할 것이다.

영문 초록

This study was performed to investigate the needs for nursing case management program of the elderly inpatients. The results of survey with 290 elderly patients who were more than 60 year old and would be soon discharged in four general hospitals are as follows. 1) 71.4% of the hospitalized elderly patients have hospitalized at least more than once for last year and the elderly patients who could perform independently all categories in ADL were 64(22.1%). 2) When Informational, Physical, and Emotional/Social Nursing Care Needs were scored from 0 to 2, the results showed 1.45, 1.35 and 1.45 respectively. 3) Level of Needs for the Case Management Program of the Hospitalized Elderly Patients The programs that the elderly patients want were telephone consulting service after discharge(71.0%), education program for patient management(64.8%), home care service after discharge(61.0%), individualized consultation(60.3%), and information provision and/or referral service on other community-based services(52.1%) orderly, and the level of needs to each program showed all high. After measuring the functional condition and nursing care needs on discharge of hospitalized elderly patients in this study, the result showed that the elderly health management is required for hospitalization period in hospitals as well as transport to local community. Therefore, It is thought that the service provision by the case management team based on the multi-disciplinary case management program model suggested in this study would be suggested as a constituent of the effective health care delivery system for the hospitalized elderly patients.
