노인입원환자의 질병유형과 양상에 관한 연구

국문 초록

우리나라 종합전문요양기관을 이용한 노인환자의 의무기록 자료를 중심으로, 1997년 1월 1일부터 동년 12월 31일까지 1년 동안 60세 이상 노인의 총 퇴원환자 50,968 진단건수를 대상으로 한국표준 질병사인 분류표에 의거 1개 진단과 2개 이상진단 시의 질병유형 및 양상에 영향을 미치는 요인을 규명하였다. 남자의 경우 신생물이, 여자의 경우 당뇨병이, 그리고 남녀 공히 백내장, 심혈관 질환이 많았다. 또한 평균 재원일수는 2개 이상 진단시 뇌경색질환이 24.3일 백내장이 4.5일이었으며, 종합전문요양기관은 사망 전 찾는 의료기관이며 단기치료를 담당하고 있음을 알 수 있었다. 정준상관분석에서 설명력이 가장 높은 결과는 권역으로 노인질병의 특성상 거주지 고착성이 강한 것을 뒷받침해주었다. 따라서 노인보건의료서비스는 병원 임상자료의 확보를 통해 구축된 데이터베이스에 기반을 두어야 하며, 지역보건사업과 연계된 사회복지서비스가 이루어져야 할 것이다.

영문 초록

This is to comprehend the data collected on the diseases of the old from inpatients' medical records in the tertiary referral hospitals in Korea, and to present the policy target to serve health care service for the old on the basis of these. The subject of investigation is the total 50,968 discharged patients of sixty or over from the tertiary referral hospitals during one year from January 1, 1997 to December 31, 1997. They find the causes influenced on the disease pattern of one diagnosis and more than two diagnoses by the KCD-3. There are more cases of the neoplasm in the male patients and diabetic in the female patients. Cataract, heart vessel diseases are largely found in both genders. The average of length of stay is to be 24.3 days in the cerebral infraction disease, and 4.5 days in cataract in the more than 2 diagnoses. It's revealed the tertiary referral hospitals is the last care center and the acute care center. The best expressive results shown in the canonical correlation analysis of the old to access is the areal specification, which means their solidity to the dwelling houses. And the health care service for the old shall be based on the data-base constructed with collecting the treatment data from the hospital. And the social welfare system should also be closely related with the community health care service.
