지역사회중심재활 모형을 기반으로 한 언어치료 접근

국문 초록

재활의 한 환경인 지역사회중심재활(CBR) 환경에서, 언어치료는 수요에 비해 전문 언어재활사(언어치료사)의 공급이 부족하다. 국내에 CBR의 모델로서의 언어치료 접근이 정리되어 있지 못하다. 특히 보건소 및 교육청과 같은 지역사회 단체가 언어재활사가 소속된 전문재활병원과 함께 참여한 문헌이 없어서 본 연구가 필요하다. 강원도의 특정 군 소재의 초등학교 저학년 학생을 대상으로 학습장애 위험성과 관련된 부모설문 및 교사 설문과 이어진 언어검사를 이용하여 3명의 학생을 그룹언어치료군으로 선정하였다. 선정과정에서 동의가 이루어진 부모(6명)에 대하여 부모교육을 시행하였다. 그룹언어치료 후 듣기이해, 문자읽기, 단락읽기 능력이 향상되었으며(p<0.05) 부모교육에 대한 만족도가 높았다. CBR을 위한 자원으로서, 다양한 지역사회 단체(병원, 보건소, 교육청 등)의 여러 직역(의사, 언어재활사, 교사 및 다양한 직역)이 공조하는 과정을 기술하여 CBR의 모델을 제시하였다. 임상적으로 타당한 결과와 함께, 언어치료 접근의 한 모델을, 그 시행 과정과 함께, 제시하여 추후 비슷한 시도에 도움이 되고자 한다.

영문 초록

Among models of community-based rehabilitation (CBR), any systematic report of the participation of speech language pathologist was not found. The purpose of this study is to demonstrate the model that a speech language pathologist participate as a CBR team member with helps of the public health center and the office of education. The screening survey for detection of the risk of development of learning disorders was done for teachers and parents of low-grade students in elementary schools of the specific province in Gangwon-Do. Via subsequent language tests, 3 students for the group therapy program was chosen. Parents education was done for the parents who consented to the participation to the program. Group therapy increased the score of auditory comprehension, sentence structure comprehension and paragraph comprehension significantly. Satisfaction to the parents education was high. We demonstrate a model system of speech therapy approach as a CBR program, which comprises the participation of many community organizations.
