상황이론에 기반 한 노인요양시설의 거주자중심케어(Resident-Centered Care) 발생경로에 관한 연구

국문 초록

본 연구는 상황이론에 근거하여 노인요양시설의 거주자중심케어 발생요인을 검증하는 것을 목적으로 한다. 거주자중심케어는 노인요양시설에서 이루어지는 케어의 관점을 시설중심이 아닌 거주노인에게 두고 이들의 가치와 선호, 욕구에 기반 한 케어방식을 의미한다. 본 연구는 이러한 거주자중심케어를 조직의 생존과 번영을 위한 혁신활동으로 이해하고 이러한 혁신활동이 어떠한 과정과 경로를 통해서 발생하게 되는지를 상황이론에 근거하여 규명하고자 한다. 이를 위해 연세대학교 사회복지연구소(2010)에서 수행중인 연구인 거주자중심(Resident-Centered) 노인요양시설에 관한 연구: 문화변화(Culture Change)의 관점과 한국적 함의 에서 수집한 데이터를 본 연구목적을 위해 활용하였다. 최종분석에 사용된 시설은 전국 노인요양시설 223개소이며 분석단위는 시설이다. 주요 분석결과를 살펴보면 외부환경동태성 중 시장ㆍ기술격변성만이 거주자중심케어에 정(+)적인 영향을 미쳤으며 조직특성에서는 보상화와 부서간 역학만이 거주자중심케어에 정(+)적으로 유의미한 영향을 미치는 것으로 분석되었다. 비록 거주자중심케어에 대한 영향력은 발견할 수 없었지만 사회환경격변성은 조직특성 및 구조의 하위차원인 보상화에 부(-)적인 영향을, 경쟁의 강도는 집중화에 정(+)적인 영향을, 반면에 부서간 역학에는 부(-)적인 영향을 미치는 것으로 확인되었다. 또한 시장ㆍ기술격변성은 최고경영층의 위기대처와 보상화, 부서간 역학에 모두 정(+)적인 영향을 미치는 것으로 나타났다. 또한 본 연구에서는 이러한 연구모형이 시설규모에 따라 어떠한 집단 간 차이를 나타내는지 검증하기 위해 다집단 분석을 추가적으로 실시하였으며 그 결과 일부 경로에서 부분적인 집단 간 차이라 존재하는 것으로 나타났다. 이와 같은 연구결과를 토대로 노인요양시설에서 거주자중심케어를 위한 구제적인 개입 방안을 제언하였다.

영문 초록

The purpose of this study is to verify what is affecting factors on Resident-Centered Care in Long-term care facilities. The Resident-Centered Care means to stress the uniqueness and worth of each elderly residents and focus perspective of care to the elderly residents not staffs and facilities. In this study, Resident-centered care is regarded as a organizational innovation for survival of organization. we investigated how Resident-centered care can be occured through the path based on Contingency theory. For this study, data of study for 「Resident centered long-term care facility: Perspective of Culture Change and Korean implications(2010)」 is used. The unit of analysis of this study is an organization and 243 out of 360 organizations responded and 223 cases were finally analyzed. The main results of this study are the following, The market-technological turbulence among Environment Dynamics and reward system, interdepartmental dynamics among organization characteristics directly had a statistically significant positive influence on Resident-Centered Care. Although there are no significant influences on Resident-Centered Care by social environment turbulence and competitive intensity, reward system were influenced negatively by social environment and centralization was influenced positively by competitive intensity. On the contrary to this, interdepartmental dynamics was influenced negatively by competitive intensity. Also Top management's emphasis and risk aversion, reward system, interdepartmental dynamics were influenced positively by market-technological turbulence. Finally based on the results of this study, we suggest specific intervention for Resident-Centered Care in Long-term care facilities.
