자궁절제술을 받은 여성의 삶의 질과 사회적 서비스 지원

국문 초록

본 연구는 자궁절제술을 받은 여성의 신체적, 심리적 변화에 따른 여성성, 자존감, 신체이미지, 자궁중요도, 상실감 등을 통하여 삶의 질을 알아보고 이들에 대한 사회적 서비스방안을 모색하는데 목적이 있다. 우리사회에서 자궁은 여성성의 상징이자 출산의 역할을 하는 것으로 보고 있다. 따라서 자궁절제술을 받은 여성은 자궁상실에 따른 신체적 변화 뿐 만 아니라 심리적 변화를 경험하게 된다. 즉 정체성의 혼란, 자존감 및 신체적 이미지약화, 그리고 상실감들을 경험하게 되며 한편으로는 자궁절제를 통해 그동안의 자궁질환으로 인한 고통에서 벗어나는 해방감을 느끼기도 한다. 그러나 자궁절제후 6개월이후 점차 상실감 등 다양한 수술후 증후군(post hysterectomy syndrome)가 나타나고 이에 따른 신체적, 심리적 고통을 경험하게 된다. 이러한 변화는 여성의 삶의 질에 영향을 미칠 수 있다. 본 연구는 자궁절제술을 받은 여성을 대상으로 여성성, 신체이미지, 자존감, 상실감, 자궁에 대한 인식, 나아가 사회적 지지로서 의료진 및 배우자의 지지를 알아보고 각 변인들이 삶의 질에 어떠한 여성을 미치는가에 대해 알아보았다. 주요 연구결과로 높은 점수의 여성성 및 자궁중요도, 낮은 점수의 신체이미지, 자존감을 보였으며 여성성, 신체이미지, 상실감이 여성의 삶의 질에 영향을 미쳤는데 이 중 상실감이 가장 영향을 미치는 것으로 나타났다. 연구결과를 기반으로 자궁절제술을 받은 여성의 삶의 질 향상을 위해서는 자궁에 대한 인식전환과 함께 신체적, 심리적 변화와 관련된 정보 및 지식을 제공하기 위한 다양한 교육 지원기관 등 사회적 서비스지원방안을 제안하였다.

영문 초록

The women who had a hysterectomy, as a matter of course, go through a lot of changes physically and psychologically. The study pays special attention to their feminity, self-respect, physical image, womb-importance, and sense of loss, apparently due to their physical and psychological changes. It, therefore, aims to examine the quality of women's lives after having a hysterectomy, and to draw up a social service measures to support them. Wombs are, in our society, not only a symbol of feminity but an organ of the body for childbirth. The women who had a hysterectomy naturally experience physical and psychological changes because of losing their wombs. They, that is, suffer from confusion of identity, weakening self-respect and physical image, and sense of loss, but on the other hand they feel like relieving from a lasting pain of womb disease. There appear another post hysterectomy syndromes such a sense of loss to them six months since a hysterectomy, so that they cannot help suffering physically and psychologically. The continuous changes may considerably affect the quality of their lives. The study, targeting the women who had a hysterectomy, surveys feminity, physical image, self-respect, sense of loss, womb-perception, and besides spouse and medical team as a social support. And then it inquires how varied elements affects the quality of their lives. It, as a result, shows that feminity and womb-importance get high points, but physical image and self-respect low points. Feminity, physical image and sense of loss affect the quality of their lives, especially sense of loss, among them, has the greatest influence. The study presents a few suggestions to improve the quality of lives for the women who had a hysterectomy. Above all, perception of womb should be converted, and information and knowledge related to physical and psychological changes must be provided. For that goal, it draws up the varied educational institutions and social service measures to support and guide them.
