임상실습의 대체교내실습 후 졸업한 간호사의 임상 적응 경험

국문 초록

본 연구는 COVID-19 팬데믹 상황에서 대체교내실습을 거쳐 졸업한 신규 간호사들의 임상 적응 과정을 심층적으로 탐구하기 위한 연구이다. 연구자들은 Strauss와 Corbin (1998)의 근거이론 연구방법을 사용하여 COVID-19 팬데믹 시기에 임상실습 반 이상을 대체교내실습으로 시행 후 300병상 이상의 종합병원에 재직 중인 신규 간호사들의 임상 적응 경험에 대한 실체이론을 도출하고자 하였다. 개방 코딩, 축 코딩 및 선택적 코딩 결과, 241개의 개념, 21개의 하위 범주 및 8개의 범주가 도출되었다. ‘수없이 깨지며 신규껍질에서 벗어나기 위해 몸부림 침’의 중심현상이 작용상호작용 전략을 통해 ‘버티고 또 버티며 제몫을 하는 간호사로 홀로 서 감’이라는 결과로 나타났다. ‘버티고 또 버티며 제몫을 하는 간호사로 홀로 서 감’의 결과는 중재적 조건인 ‘빠른 임상 적응에 대한 의지’ 는 의지정도와 적응방식에 따라 주도적 적응형, 지지망 의지형. 현실적 수용형으로 세분화 되었다. 본 연구는 ‘수없이 깨지며 신규껍질에서 벗어나기 위해 몸부림 침’의 중심현상을 중심으로 COVID-19 팬데믹 상황에서 대체교내실습을 거쳐 졸업한 신규 간호사들의 임상 적응 과정을 설명하는 실체이론을 제시하였고, COVID-19와 같은 팬데믹 상황에서 부족한 임상실습 경험을 보완하기 위한 대체교내실습 및 임상 적응 증진을 위한 프로그램 개발의 기초 자료를 제공하였는데 의의가 있다.

영문 초록

This research was conducted to deeply explore the clinical adaptation processes of new nurses who graduated after undergoing alternative on-campus practicums during the COVID-19 pandemic in Korea. The researchers used Strauss and Corbin’s (1998) grounded theory research method to derive a specific theory regarding the clinical adaptation experiences of newly graduated nurses working in hospitals with over 300 beds, after conducting more than half of their clinical practicum as on-campus practicum during the COVID-19 pandemic. Through the processes of open coding, axial coding, and selective coding, 22 concepts, 21 subcategories, and 8 categories were identified. The central phenomenon of ‘Struggling endlessly to break free from the new shell and standing alone as a nurse who perseveres and does their part’ emerged through interactive strategies. The results were further subdivided into proactive adaptive, supportive willful, and realistic accommodating types, depending on the degree and style of determination for rapid clinical adaptation. This study provides a theoretical framework that explains the clinical adaptation process of newly graduated nurses who completed alternative on-campus clinical practicum during the COVID-19 pandemic, with the central phenomenon of ‘Struggling endlessly to break free from the new shell.’ It also serves as foundational data for the development of programs aimed at improving on-campus practicum and clinical adaptation in situations like COVID-19, where clinical practicum experiences are lacking.
