국문 초록

본 연구는 중소병원 간호사를 대상으로 근접 오류보고 의향 정도 및 근접 오류보고 의향에 영향을 미치는 요인을 파악함으로써, 중소병원의 근접 오류보고 향상을 위한 관리전략을 마련하는데 기초자료를 제공하고자 시도되었다. 2019년 6월20일부터 7월20일까지 G도 소재 K시의 4곳의 중소병원에 재직 중인 간호사 205명에게 설문조사를 실시하였다. 수집된 자료는 SPSS Windows 24.0를 활용하였으며, 도구의 신뢰도는 Cronbach's α로 측정하였고, 기술통계, 독립표본t검정, 일원배치분산분석, 쉐페 사후검정, 피어슨상관계수, 다중회귀분석을 실시하였다. 연구결과, 대상자의 근접 오류보고 의향은 평균 73.41점(100점 만점), 안전 동기는 평균3.22점(5점 만점), 상사 신뢰는 평균 4.01점(5점 만점) 및 조직 의사소통 만족은 평균 3.31점(5점 만점), 환자 안전 침묵은 평균 2.27점(5점 만점)이었다. 대상자의 근접 오류보고 의향과 환자 안전 침묵(r=-.395, p<.001), 환자 안전 동기(r=.406, p=.001), 상사 신뢰(r=.149, p=.033), 조직 의사소통 만족(r=.300, p<.001)간의 상관관계를 보였다. 대상자의 근접 오류보고 의향에 영향을 미치는 요인은 안전 동기(β=.328, p<.001), 환자 안전 침묵(β=-.285, p<.001), 안전사고 보고 절차인지(β=.142, p=.021)순이었고, 설명력은 26.2%이었다. 결론적으로 중소병원 간호사의 근접 오류보고 의향에 영향을 미치는 요인은 안전 동기, 환자 안전 침묵 및 안전사고 보고 절차인지였다. 따라서 중소병원 간호사의 근접 오류보고 의향을 높이기 위해서는 환자 안전교육을 통한 안전 동기를 증진시키고 개인 및 조직의 의사소통 훈련프로그램을 적용하여 환자 안전 침묵을 낮추어야 한다. 그리고 중소병원 간호사들이 안전사고 보고 절차에 대하여 잘 알 수 있도록 보고 절차를 명확히 하고 간소화하여 쉽게 접근할 수 있도록 해야 한다.

영문 초록

This study conducted to provide basic data on developing management and intervention strategies for improving near miss reporting in small and medium-sized hospitals by identifying the degree of willingness of nurses to report near miss and factors influencing their willingness to report such occurrences. The participants were 205 nurses. who were recruited from the 4 small and medium-sized hospitals with more than 200 beds in K-city, Gyeongnam Province. The structured questionnaire was self-administered from June 20 to July 20, 2019. Data were analyzed through the SPSS/WIN 24.0 program using descriptive statistics, Independent t-test, one-way ANOVA, Scheffé test, Pearson’s correlation coefficient, and stepwise multiple regression. According to the results of this study, The degree of participants’ willingness to report near miss was 73.41 out of 100 points. The degree of patient safety silence was 2.27 out of 5 points . The degree of the safety motivation was 3.22 out of 5 points . The degree of faith in supervisor was 4.01 out of 5 points . The degree of satisfaction with the organization's communication was 3.31 out of 5 points . The factor of patient safety motivation(β=.328, p<.001), patient safety silence(β=-.285, p<.001), and safety accident reporting procedure(β=.142, p=.021) were impact on the willingness to report the near-miss level of nurses. The total explanatory power accounted for 26.2%. In conclusion, factors that affect the willingness of nurses to report near misses in small and medium-sized hospitals were safety motivation, patient safety silence, and safety accident reporting procedures. Therefore, in order to increase the willingness of nurses to report near miss in small and medium-sized hospitals, safety incentives should be enhanced through patient safety education, and communication training programs for individuals. Moreover, organizations should be applied to reduce patient safety silence. Additionally, the reporting procedure should be clarified and simplified so that nurses in small and medium-sized hospitals understand w the safety accident reporting procedure well and can access it easily.
