국문 초록

2019-2021년 학술지 􋺷의료인문학􋺸(Medical Humanities)의 지면을 통해 “질병 없는 질환”(illness without disease)으로서의 만성피로증후군(chronic fatigue syndrome) 혹은 근육통성 뇌척수염(myalgic encephalomyelitis)과 관련해 벌어진 논쟁은 1950-60년대 이래 의료사회학과 의철학의 영역에서 질병(disease)과 질환 (illness) 개념을 둘러싸고 이루어진 논의에 대한 관심을 다시 불러일으켰다. 이 글은 병과 건강에 대한 자연주의(naturalism)와 규범주의(normativism) 관점 간의 논쟁의 산물인 “질병 없는 질환” 개념을 통해 질병과 질환 개념의 사이의 간극이 생겨나게 된 역사적 맥락과 이론적 배경을 살펴보고자 한다. 특히 이 글은 <정신질환 진단통계편람> (Diagnostic and Statistical Manual of Mental Disorders)이 다양한 방식으로 정의하는 정신질환들과 현대의학기술의 발전에 따라 새롭게 발견(발명)된 질병들을 검토함으로써 의학과 정치의 공모를 전제한 규범주의 관점의 이론적, 현실적 유효성을 확인하고, 이를 통해 환자의 말과 고통에 관심을 기울이는 질환 중심의 치료가 필요함을 강조하고자 한다.

영문 초록

Recent controversy in Medical Humanities over the concept of “illness without disease” in relation to chronic fatigue syndrome/myalgic encephalomyelitis(CFS/ME) rekindles the debate on the disease-illness dichotomy took place since the 1950s-60s in the disciplines of medical sociology and philosophy of medicine. Beginning with “illness without disease,” a product of the heated debates on the naturalist-normativist view of disease, this essay aims to trace out the historical and theoretical contexts in which the discrepancy between the concepts of disease and illness arises. Examining selected diseases from the mid-twentieth century to the present, ranging from the mental diseases variously defined by the editions of Diagnostic and Statistical Manual of Mental Disorders to the newly found (or invented) diseases with the development of medical knowledge and instruments, this essay will confirm the theoretical and practical validity of normativism, a view that acknowledges the collusion between medicine and politics, and stress the need for illness-oriented rehabilitative treatment which cares more for the patient’s pains and needs.
