국문 초록

모두의 건강을 보장하기 위한 국가와 사회의 노력에서 임신중지를 비롯한 성･재생산 건강 의제는 지속해서 배제되어 왔다. 이 연구는 지금까지 정부의 정책에서 여성건강이 어떻게 다루어져 왔는지 살펴보고 보건의료체계 개념을 소개하며, 이를 토대로 성･재생산 건강보장을 위한 보건의료체계의 과제를 제시하였다. 지난 20여 년 사이 여성건강에 대한 정부정책은 모성건강의 협소한 범위를 벗어나 차츰 그 영역을 확대하고 있지만 여전히 여성의 건강을 재생산하는 몸으로 환원하고, 개입을 위한 적절한 책무 구조를 갖추고 있지 못하다. 이는 불평등한 의사결정과 자원배분 구조가 지속되고, 성차별적 실천에 대한 직접 개입보다 인식과 규범 변화에 치우치고 있는 상황과 관련이 있다. 모두에게 안전한 양질의 임신중지를 보장하고, 성･재생산 건강을 향상하기 위해서는 보건의료체계 전반의 변화가 필요하다. 여기에서는 세계보건기구의 보건의료체계 개념틀을 활용해 포괄적 성･재생산 건강보장을 위한 보건의료체계의 과제를 모색하였다. 형법상 낙태죄 폐지와 임신중지 합법화는 여성의 성･재생산 건강 의제를 진전하기 위한 중요한 기회이자 계기이다. 여성들이 직접 열어젖힌 정치적 기회를 놓치지 않고 여기에 개입하기 위해 학제적 협력을 강화하고, 여성들의 경험에서 출발하는 정책적 지식을 만들어 나가야 한다.

영문 초록

Sexual and Reproductive Health, including abortion services, had long been excluded from the government’s effort to ensure “health for all” in South Korea. This study is a narrative review that analyzes previous women’s health policy, introduces the health system conceptual framework, and presents the health system’s forthcoming tasks to ensure sexual and reproductive health and rights. In the last two decades, women’s health policy had escaped from the narrow scope of maternal health, but still, it reduces women’s health into the reproductive body, and the adequate accountability structure for women’s health is still absent. The policy focuses on awareness-raising and changing social norms rather than material condition of women’s health, such as health care service production or challenging the unequal distribution of resources. A health system approach is essential to assure safe abortion and improve access to sexual and reproductive health for everyone. We explored the health system’s future task, utilizing WHO’s six building block framework for health system strengthening. Decriminalization of abortion and legalization of abortion is a significant opportunity to advance the sexual and reproductive health agenda. Scholars should not dissipate the political window from the women’s movement, producing policy alternatives that start from women’s own experience, which requires constructive inter-disciplinary collaboration between public health and women’s studies.
