소아에서 휴대형 자동굴절검사기와 탁상형 자동굴절검사기, 망막검영법의 정확성 비교

국문 초록

목적: 소아에서 휴대형 자동굴절검사기와 탁상형 자동굴절검사기, 망막검영기로 굴절이상을 측정하여, 휴대형 자동굴절검사기의 정확성과 효용성에 대해 알아보고자 하였다. 대상과 방법: 90명의 소아에게 조절마비 전에 탁상형 자동굴절검사기(Humphrey ARK 599, Humphrey Inc., Dublin, CA, USA)와 휴대형 자동굴절검사기(Nikon Retinomax K-plus 2, Nikon, Tokyo, Japan)를 이용하여 굴절이상을 측정하고, 이 중 30명은 조절마비 후에 위의 두 기기와 망막검영기로 굴절검사를 하였다. 각 검사의 구면도수, 난시도수, 난시축, 구면렌즈대응치를 분석하였다. 결과: 휴대형 자동굴절검사기는 탁상형 자동굴절검사기에 비해 조절마비 전의 구면도수와 구면렌즈 대응치가 각각 0.70 D, 0.68 D 낮게 측정되었으며(p=0.000), 난시도수와 난시축은 차이가 없었다. 조절마비 후 두 검사기기와 망막검영기는 모두 높은 상관관계를 보였고 구면도수와 구면렌즈대응치도 유의한 차이가 없었다. 결론: 휴대형 자동굴절검사기는 협조가 잘 되지 않는 소아에게 빠르고 정확하게 굴절이상을 측정하기 위해 유용하게 사용될 수 있을 것으로 생각한다.

영문 초록

Purpose: To evaluate the accuracy and utility of the hand-held autorefractor, compared with on-table autorefractor and retinoscopic refraction in children. Methods: Ninety children underwent autorefraction by hand-held autorefractor (Nikon Retinomax K-plus 2, Nikon, Tokyo, Japan) and on-table autorefractor (Humphrey ARK 599, Humphrey Inc., Dublin, CA, USA) without cycloplegia. Among them, 30 children underwent both two autorefraction and retinoscopic refraction under cycloplegia. We compared sphere, cylinder, cylinder axis, and spherical equivalent in each test. Results: Measurement data of hand-held autorefractor was 0.70 D more myopic in sphere, and 0.68 D more myopic in spherical equivalent than that of on-table autorefractor without cycloplegia. But there was no significant difference in cylinder and its axis on both noncycloplegic and cycloplegic eyes between the two autorefractors. Compared with the cycloplegic refraction measurement, we found no significant difference from the measurement data of Retinomax. Analysis of the measurement data showed coefficient of high correlation between the two autorefractors with or without cycloplegia. Conclusions: The hand-held autorefractor is a useful instrument to screen refractive errors in young, uncooperative children.
