공막연화증에서 시행한 양막이식술에서 재이식의 위험인자에 대한 장기 관찰 분석

국문 초록

목적: 익상편절제술 후 발생한 공막연화증으로, 상피결손과 공막결손을 복구하기 위해 양막이식술을 시행받았으나, 양막 재이식이 필요했던 환자에서 재이식의 위험인자에 대해 분석하고자 하였다. 대상과 방법: 양막 이식 수술 후 6개월 이상 추적관찰하였던 14명 15안의 의무기록 후향적 분석을 하였다. 수술 후 재수술이 필요했던 4안과 그렇지 않았던 11안에서, 환자의 성별, 나이, 익상편절제술부터 공막연화증 발생까지 걸린 기간, 수술 당시의 염증정도, 임박한 천공유무, 칼슘판 유무, 미생물검사결과, 공막이식술 실시여부, 수술 후 자가혈청 사용여부, 수술 중 측정한 공막결손의 크기를 비교하였다. 결과: 동시 공막편 이식여부 및 기타 위험인자는 두 그룹에서 유의한 차이를 보이지 않았으며(p=0.538), 수술 중 측정한 공막결손의 크기만 두군에서 유의하게 차이를 보였다(p=0.012). 공막편이식을 하지 않은 양막이식만 시행한 소집단분석시에도 공막결손 크기 외에 위험인자들은 유의한 차이를 보이지 않았다. 결론: 공막연화증에서 공막편 이식여부는 재이식에 영향을 끼치지 않으며, 수술 중 측정한 공막결손의 크기는 양막이식술 후 재이식의 위험인자로 추정된다.

영문 초록

Purpose: To investigate the risk factors of re-transplantation of amniotic membrane to repair epithelial and scleral defect in scleromalacia after pterygium excision. Methods: The medical records of fifteen eyes in 14 patients who had followed-up at least 6 months after amniotic membrane transplantation were retrospectively reviewed. We compared presumable risk factors of re-transplantation group (n=4) to those of control group (n=11), such as sex, age, onset of scleromalacia after pterygium excision, activity of scleritis at the time of operation, impending perforation, existence of calcium plaque, microbe culture result, the size and area of sclera defect, simultaneous application of scleral patch graft, postoperative use of autoserum. Results: There was no statistical significance in the concomitant application of scleral patch graft (p=0.536) and other risk factors between two groups except scleral defect size (p=0.012). The only significant risk factor was scleral defect size in sub-group analysis which had undergone amniotic transplantation only without scleral patch graft. Conclusions: Scleral patch graft might not be involved in the risk of re-transplantation, of amniotic membrane, while the size of scleral defect might be the risk factor to predict amniotic membrane re-transplantation after repair of scleromalacia.
