안구건조증에서 정량적 눈물액 삼투압 측정을 통한 레스타시스Ⓡ의 치료 효과

국문 초록

목적: 건성안에서 눈물액 삼투압 측정하여 레스타시스Ⓡ (Allergan Inc., Irvine, CA, USA)의 치료효과를 알아보았다. 대상과 방법: 건성안을 진단받은 환자 50명, 100안을 대상으로 레스타시스Ⓡ와 인공누액을 사용하며, LASEK 수술을 받은 군 22명 44안과 안과적 수술을 받지 않은 군 28명 56안에 대해 전향적 실험연구를 하였다. 설문조사, 쉬르머 검사, 눈물막파괴검사 및 눈물액 삼투압 검사는 치료 전과 치료 후 1, 2, 3개월 및 6개월에 시행되었다. 결과: 설문조사에서 안구 증상은 두 군 모두 레스타시스Ⓡ 점안 후 1개월째부터 호전되기 시작하였고, 쉬르머 검사, 눈물막파괴검사 및 눈물액 삼투압도 유의하게 개선되었다(p＜0.05). 시기별 평균 삼투압변화는 치료시간과 유의한 상관성을 보였다(p＜0.05). 두 그룹 모두에서 설문조사와 눈물액 삼투압수치는 감소했으며, 쉬르머 검사와 눈물막파괴시간 수치는 증가하였다. 두 그룹의 평균 삼투압은 레스타시스Ⓡ 점안 후 1, 2개월 및 6개월에서 유의한 차이를 보였다. 결론: 눈물액 삼투압 측정법은 건성안의 진단 및 치료효과 평가에 객관적이고 재현성 높은 검사로서 사용될 수 있을 것이다.

영문 초록

Purpose: To evaluate the effect of 0.05% cyclosporine emulsion on tear osmolarity measured by TearLabTM(OccuLogix, San Diego, CA, USA) in dry eye syndrome. Methods: From patients diagnosed with dry eye syndrome, 0.05% cyclosporine emulsion and lubricant eye drop were administered to 100 eyes of 50 patients. The prospective study was enrolled with 44 eyes of 22 patients who had been performed with laser ephithelial keratomileusis (LASEK Group) previously and 56 eyes of 28 patients with no previous ocular surgical history (Non LASEK Group). From each patient, symptoms questionnaire based on ocular surface disease index (OSDI), Schirmer test with anesthesia, Tear film break up time (TBUT) and osmolarity were evaluated. The clinical parameters and symptoms were checked before and 1, 2, 3, and 6 months after treatment. Results: Ocular symptoms evaluated by OSDI symptoms questionnaire in LASEK and Non-LASEK group started to improve from 1 month after use of RestasisⓇ (Allergan Inc., Irvine, CA, USA) and Schirmer test, TBUT and osmolarity improved significantly (p＜0.05). The changes of average values with time had a high correlation from 0.586 to 0.993 (p＜0.05). Both OSDI and Tear osmolarity values decreased, and Schirmer test and TBUT values increased in each two group. The average values of tear osmolarity were statistically different at 1 month, 2 months and 6 months after administration of RestasisⓇ between two groups. Conclusions: The measurement of osmolarity of tear film can be used not only to diagnose the dry eye diease, but also to evaluate the therapeutic effect of RestasisⓇ as more objective and reproducible tests.
