망막정맥폐쇄에 의한 황반부종 환자에서 유리체강 내 베바시주맙 주입술과 트리암시놀론 주입술의 치료 효과 비교

국문 초록

목적: 망막정맥폐쇄에 의한 황반부종 환자에서 유리체강 내 베바시주맙 주입술과 트리암시놀론 주입술의 치료효과를 비교하고자 하였다. 대상과 방법: 분지망막정맥폐쇄와 중심망막정맥폐쇄로 진단받고 유리체강 내 베바시주맙 또는 트리암시놀론 주입술을 시행받은 69명 69안을 대상으로 시술 전후 최대교정시력과 중심황반두께의 변화를 후향적으로 분석하였다. 결과: 분지망막정맥폐쇄와 중심망막정맥폐쇄 환자군 모두에서 트리암시놀론과 베바시주맙의 유리체강 내 주입에 따른 시력호전과 중심황반두께의 감소를 확인할 수 있었으며, 이는 통계적으로 유의한 결과를 나타냈다(p＜0.05). 그러나 트리암시놀론과 베바시주맙의 두 가지 치료법에 따른 치료효과는 통계적으로 유의한 차이를 보이지 않았다(p＞0.05). 결론: 황반부종을 동반한 분지 및 중심망막정맥폐쇄 환자에서 유리체강 내 트리암시놀론과 베바시주맙의 주입술은 모두 단기적으로 시력호전과 중심황반두께의 감소를 기대할 수 있는 치료법이며, 향후 병합주입 혹은 반복주입에 따른 장기간의 연구가 추가적으로 필요할 것으로 사료된다.

영문 초록

Purpose: To evaluate the treatment effect of intravitreal bevacizumab injections in patients with retinal vein occlusion, and to compare the clinical outcomes of bevacizumab treated patients with those of Triamcinolone Acetonide (TA) treated patients. Methods: A retrospective chart review will be conducted on patients who are identified with CRVO or BRVO, and are treated by intravitreal bevacizumab or TA injection. Results: There were 47 patients diagnosed with BRVO. Seventeen eyes were treated with intravitreal bevacizumab injections and 30 eyes with intravitreal TA injections. In both group, there were improvements in visual acuity at 1-, 2-, and 3-month follow-up, and declination of central macular thickness at 1-month follow-up (p＜0.05). Twenty two patients were diagnosed with CRVO, and treated with bevacizumab (8 patients) and TA (14 patients). In both two groups, mean visual acuity was improved in all follow-up periods (p＜0.05). No significant differences of the treatment effect between bevacizumab and TA were found in this study. Conclusions: The visual benefits of intravitreal bevacizumab for macular edema are almost equvalent to those of intravitreal TA injections. On the basis of the results of this study, intravitreal injection of bevacizumab may be an alternative or even first-line treatment for RVO patients with the risk of steroid complications.
