보건진료소를 방문한 노인의 인지기능에 영향을 미치는 건강관련요인

국문 초록

본 연구는 일부 농촌 지역에 거주하는 재가노인의 인지기능에 영향을 미치는 요인을 건강관련 변수인 식습관, 일상생활수행능력, 음주 및 흡연을 중심으로 파악하기 위한 서술적 조사 연구이다. 연구 대상자는 K지역의 M, S, D 보건진료소를 방문한 재가 노인 156명이다. 자료수집 기간은 2015년 9월 7일부터 2015년 12월 22일까지였다. 연구변수간의 관계는 Pearson s 상관계수(Pearson s correlation coefficient)로 분석하였고, 인지기능에 영향을 미치는 요인으로 연령, 학력, 흡연여부와 건강관련 변수인 식습관, 일상생활수행능력, 음주, 흡연을 독립변수로 투입하여 단계적 다중회귀분석(stepwise multiple regression)을 통해 분석하였다. 연구 결과 대상자의 인지기능은 평균 27.56점(30점 만점)으로 높았으며, 88.5%가 정상범위에 속하였다. 대상자의 인지기능과 음주는 부적관계가 있었고, 인지기능과 일상생활수행능력 간에는 정적 관계가 있었다. 인지기능의 예측변인은 연령, 학력 및 일상생활수행능력으로 나타났고 이들 변인은 인지기능의 34.3%를 설명하였다. 따라서 농촌지역 노인의 인지기능을 유지 및 향상하기 위해 인지기능에 대한 주기적 평가 및 관리가 요구된다. 또한 일차보건의료기관인 보건진료소를 중심으로 인지기능향상 접근 시 일상생활수행능력을 포함시킬 필요가 있다.

영문 초록

The purpose of this study is to identify the relationship among eating habits, ADL, CAGE, nicotine dependence and cognitive function of the elderly in primary health care posts. 156 people over 65 years old living in K area were interviewed, from September 7th to December 22nd, 2015. The outcomes were measured by the Korean version mini mental state examination instrument (MMSE-K), activity of daily living (ADL), cut down, annoyed, guilty, eye-opener (CAGE), and Fagerstrom test for nicotine dependence (FTND). Data were analyzed using Pearson correlation coefficients and stepwise multiple regression. Cognitive function score of the subjects was 27.56, and 88.5% of them were within normal range. There was a negative correlation between cognitive function and CAGE, but there was a positive correlation between cognitive function and ADL, and between FTND and CAGE. Age, education level, and ADL were identified as predictors of cognitive function, which explained 41.3% of total variance of cognitive function. When we prepare intervention programs for the elderly living in rural area, we need to consider age, educational level, and ADL. In addition, programs for health promotion activities should be developed to maintain and increase current cognitive function.
