독거노인과 비독거노인의 인지기능 저하에 미치는 영향요인

국문 초록

본 연구의 목적은 지역사회에 거주하는 독거노인과 비독거노인 집단의 인지기능 저하에 영향을 미치는 요인을 파악하기 위하여 고령화연구패널조사(the Korean longitudinal study of ageing) 중 2012년 제 4차 기본조사의 원시 자료를 분석한 2차 자료 분석 연구이다. 조사에 응답한 생존패널 중 65세 이상의 노인 총 4.134 명의 자료가 분석에 포함되었다. 대상자의 인지기능은 K-MMSE, 우울증은 CES-D 10, 일상생활수행능력 및 도구적 일상생활수행능력은 K-ADL 및 K-IADL을 사용하여 측정되었다. 자료분석에는 Rao-Scott chi^2 test 및 로지스틱 회귀분석 방법이 사용되었다. 두 집단의 인지기능 저하에 공통적으로 영향을 미친 요인은 고령, 낮은 학력, IADL에 제한이 있는 경우와 적은 친구 만남 횟수 등이었다. 인구사회학적 요인을 보정했을 때 독거노인 집단의 인지기능 저하에 유의한 영향을 미친 요인은 적은 친구만남 횟수, 정기적인 신체운동을 하지 않는 경우, IADL에 제한이 있는 경우 등이었으며, 비독거노인 집단에서는 우울증, 낮은 주관적 건강상태, 적은 친구만남 횟수, IADL과 ADL에 제한이 있는 경우 등이 포함되었다. 본 연구결과, 독거노인과 비독거노인의 인지기능 저하에 서로 다른 요인이 영향을 미치는 것으로 나타났다. 따라서 두 집단의 인지기능을 유지하거나 저하를 예방하기 위하여 맞춤형 중재전략을 개발하기 위한 연구가 필요하다.

영문 초록

This study aimed to identify the factors influencing cognitive impairment in community-dwelling older adults living alone and those living with others using the 4th basic survey of the Korean longitudinal study of ageing (KLoSA) 2012. The study sample consisted of 4,134 persons aged 65 years old and over who participated in the KLoSA 2012. Measurements included the K-MMSE for cognitive function, the CES-D 10-item scale for depression, the K-ADL and the K-IADL. Logistic regression analyses were conducted to identify factors influencing cognitive impairment in two groups. The common factors that significantly influenced cognitive impairment in both groups were old age, low education level, limited IADL, and infrequent visiting with friends. When adjusted for demographic characteristics, not performing regular exercise were significant factors found only in the living-alone group whereas limited ADL, the presence of depression, and low subjective health status were the significant factors found only in those living with others. Further studies need to be conducted regarding development of tailored nursing and treatment intervention strategies to prevent or delay cognitive impairment for these two groups.
