암환자 배우자의 외상 후 성장에 영향을 미치는 요인에 관한 연구

국문 초록

암의 영향은 환자에게 국한되지 않고 그들의 지지원인 배우자에게 영향을 미치게 된다. 이들은 생의 전반에 걸쳐 심각한 디스트레스를 경험하게 되지만 그러한 역경 속에서도 외상 후 성장을 경험하는 것으로 보고되고 있다. 이에 본 연구는 암환자 배우자의 외상 후 성장을 예측하는 요인을 밝혀 이들을 돕는 서비스 마련에 기초를 제공하고자 수행되었다. 이를 위해 선행연구를 토대로 암환자 배우자의 외상 후 성장에 대한 예측요인을 검토하였고 인구사회학적 요인, 스트레스 요인으로 돌봄 부담, 부부관계요인으로 부부적응, 환경적 요인으로 사회적 지지 변인을 도출하여 이들 변인의 상대적 영향력을 검토하였다. 암환자 배우자 203명을 대상으로 조사를 수행하였으며 연구결과는 다음과 같다. 위계적 회귀분석을 통해 외상 후 성장에 영향을 미치는 요인들의 상대적 영향력을 분석한 결과, 인구사회학적 요인 중 병기, 돌봄 부담 중 경제적 부담이 외상 후 성장이 유의한 정적 관계에 있었으며, 부부적응의 하위요인 중 결혼만족감은 외상 후 성장과 유의한 부적 관계를 보였고, 부부응집도와는 정적으로 유의한 관계를 보이는 것으로 나타났다. 사회적 지지는 외상 후 성장의 가장 강력한 예측요인인 것으로 나타났으며 사회적 지지의 하위요인 중 도구적 지지가 외상 후 성장이 유의한 정적 상관관계가 있는 것으로 나타났다. 이러한 결과를 토대로 후속 연구와 사회복지 서비스 마련에 대한 제언을 제시하였다.

영문 초록

Effect of the cancer is not limited to the patients but to their spouses as well. They experience serious distress throughout their life but despite this hardship, it is reported that they experience posttraumatic growth. This research aims to find predictors of posttraumatic growth in spouses of cancer patients to provide the basics for the services that help them. For this research, correlates of the posttraumatic growth for spouses of cancer patients was investigated based on the prior research. Demographic variables, care－burden as the stress factor, marital adjustment as the marital relationship factor, and social support variable for environment factor, were deducted to examine the relative influence of each variable. The self－answering survey was administered to the 203 cancer patients’ spouses for the research. The result of the survey was as follows：First, analysis of the relative effects of the factors that influence posttraumatic growth through hierarchy multiple regression, has shown that period of sickness from demographic variables and financial burden from care－burden had significant positive effects on posttraumatic growth. Second, it was also shown that satisfaction regarding marriage had significant negative effects on the post traumatic growth, while marital cohesiveness had significant positive effect. Third, it was shown that social support had the strongest predictable variable for posttraumatic growth, and finally, out of the social support, instrumental support was shown to have most significant positive effect. Based on these results, the practical implications for the posttraumatic growth of spouses of cancer patients were discussed.
