소아 편두통의 우울 및 불안성향

국문 초록

목적: 소아 청소년 편두통 환자들에게서 동반 될 수 있는 우울, 불안성향의 유병률을 알아보는데 본 연구의 목적이 있다. 방법: 2012년 3월부터 2015년 12월까지 두통을 주소로 명지병원 소아청소년과 외래를 방문하거나 입원한 만 5세 이상 18세 이하의 환자 451명 중에서 국제두통질환분류법에 의해 편두통으로 진단 된 환자 196명의 의무기록을 후향적으로 조사하였다. 동반되는 우울 및 불안성향의 정도를 평가하기 위해 각각 CDI 및 RCMAS 검사를 사용하였다. 결과: 편두통으로 인하여 일상 생활의 지장을 받는 정도는 두통의 빈도 및 심한 정도와 통계적으로 유의미한 연관성이 있었다. 우울이나 불안성향의 평가에 사용한 임계치인 13점 이상 혹은 15점 이상의 CDI, 19점 이상의 RCMAS 점수를 보인 환자들의 비율은 각각 26.5%, 23.5%, 31.1%였다. t-test, ANOVA 분석상 CDI 점수는 연령과 유의미한 연관성이 있었고 RCMAS는 연령과 연관성을 보이지 않았다. 로지스틱 회귀분석에서는 연령과 CDI, RCMAS는 통계적으로 유의미한 연관성은 없었다. 결론: 연구에 참여한 소아 청소년 편두통 환자들에서 CDI, RCMAS의 임계치 점수 이상의 점수를 보인 환자의 비율은 CDI, RCMAS를 우울, 불안성향의 선별도구로 사용한 타 연구에서 기술 된 일반 소아 청소년 인구에서의 비율보다 더 높은 수치를 나타내었다. 이는 소아 청소년 편두통 환자들에서 일반인구대비 더 높은 비율로 우울과 불안성향이 동반될 수 있는 가능성이 있음을 시사한다.

영문 초록

Purpose: The purpose of this study is to investigate depressive and anxious tenden-cies in pediatric migraine patients. Methods: We retrospectively reviewed medical records of 196 patients who visited our hospital for headache and diagnosed with migraine according to the Internatio-nal Classification of Headache Disorders-III (ICHD-III) from March 2012 to December 2015. All patients completed the tests of the Children’s Depression Inventory (CDI) and Revised Children’s Manifest Anxiety Scale (RCMAS). We defined a CDI score of ≥13 or ≥15 and a RCMAS score of ≥19 as clinically significant. Results: The mean age of the patients was 11.9±3.0 years. The ratio of male to female was 1:1.5, and 40.8% were male (n=80) and 59.2% were female (n=116). Degree of daily life impairments was significantly correlated with migraine frequency and in-tensity (P=0.041, 0.000, one-way analysis of variance (ANOVA)). Age was significantly correlated with CDI score (P=0.038, ANOVA), but it was not correlated with RCMAS score. Age was not significantly correlated with CDI or RCMAS scores in logistic regres-sion analysis. The percentages of patients who had clinically significant CDI (≥13 or ≥15) or RCMAS (≥19) scores were 26.0% (51 patients, Score of CDI≥13) and 23.5% (46 patients, Score of CDI≥15) for CDI and 31.1% (61 patients) for RCMAS. Conclusion: The percentage of patients who had clinically significant CDI or RCMAS scores was higher than the range of prevalence rates yielded from studies in other nations that had also employed the CDI or RCMAS as an estimate of overall depres-sion or anxiety levels. This result suggests that pediatric migraine patients might have more depressive and anxious tendencies than the general pediatric population.
