대화와 그림설명 과제를 통한 브로카 실어증 환자의 자발화 산출 특성

국문 초록

본 연구는 브로카 실어증 환자의 과제에 따른 자발화 산출 특성을 살펴보는 것을 목적으로 하였다. 연구대상은 중등도 이상의 브로카 실어증 환자 10명과 일반 성인 10명으로 대화와 그림설명 과제로 자발화를 수집하여 11가지 변인(Correct Information Unit; CIU 비율, 발화당 음절수, 단어수, 내용어수, 초당 음절수, 어절당 반복수, 간투사수, 음소착어수, 의미착어수, 수정수, 도치수)으로 분석하였다. 본 연구의 결과는 다음과 같다. 첫째, 집단 간 차이는 일반 성인에 비해 브로카 실어증 환자의 CIU 비율이 낮았으며, 발화당 음절수, 단어수, 내용어수 및 초당 음절수는 브로카 실어증 환자들이 일반 성인에 비해 유의하게 적었다. 반면, 어절당 반복수, 간투사수, 수정수와 음소착어수는 일반 성인에 비해 브로카 실어증 환자에게서 유의하게 많았다. 그러나 어절당 의미착어수와 도치수에서는 집단에 따른 차이가 유의하지 않았다. 다음으로 과제에 의한 차이는 대화에 비해 그림설명 과제에서의 CIU 비율이 유의하게 낮았으며, 어절당 수정수 및 반복수는 대화 과제에 비해 그림설명 과제에서 유의하게 높았다. 마지막으로 집단과 과제의 상호작용 효과는 CIU 비율에서만 유의하게 나타났다. 이러한 결과는 브로카 실어증 환자의 비유창한 자발화 산출 특성을 잘 반영한 것으로 임상 현장에서 브로카 실어증 환자의 과제에 따른 자발화 평가와 치료를 위한 기초자료로 활용가능하다.

영문 초록

The purpose of this study was to investigate the characteristics of spontaneous speech of Broca's aphasic patients by tasks. Spontaneous speech samples were collected from 10 normal adults and 10 Broca's aphasic patients and they were analyzed by using 11 variables by interview and picture description tasks. The results of this study are as follows: First, the normal adults had higher performances on CIU ratio, SPU(the number of syllables per utterance), WPU(the number of words per utterance), CWPU(the number of content words per utterance), SPS(the number of syllables per second) than Broca's aphasic patients during the two tasks. Broca's aphasic patients, however, had higher numbers for their FBW(the number of fillers per word), RPW(the number of repetitions per word), PPW(the number of phonemic paraphasias per word), and MPW(the number of modification per word). And, there were no significant differences in SPPW(the number of semantic paraphasias per word) and IPW(the number of inversions per word) between the two groups. Second, CIU ratio of interview was higher than picture description task, and the RPW and MPW in picture description had higher numbers than interview task. Finally, the interaction effect only appeared on the CIU ratio. These results suggested that Broca's aphasic patients had problems with word retrieval, slow tempo and non-fluent speech.
