치매 노인 비공식 보호의 비용에 관한 연구

국문 초록

본 연구의 목적은 비공식 부양지가 치매노인을 부양함으로써 입게되는 경제적 부담을 추정한 다음, 추정한 결과에 대한 정책적 합의를 도출하는 것이다. 65세 이상 치매노인을 수발하고 있는 주부양자들을 대상으로 설문조사를 하여 회수된 설문지 중에서 사용 가능한 81부가 본 분석 표본으로 선정되었다. 분석결과를 토대로 다음과 같은 정책적 합의에 대하여 토의할 수 있다. 첫째, 치매 노인 부양 비용에서 무급 노동 비용의 비중이 가장 높다는 것이다. 치매 부양자가족이 직접 부담하는 비용인 치료 및 간호에 소요되는 금전적 비용은 치매환자당 월평균 289.000원, 심리적 비용은 월평균 24.000원인데 비해 간접 비용인 부양시간 비용은 493.500원이나 된다. 이러한 맥락에서 정부는 치매가족에 대한 자원을 고려할 때 부양시간을 가장 우선적 다를 필요가 없다. 둘째, 증중 치매노인의 경우, 가족이 보호하는 비공식 보호 비용이 노인전문요양 시설의 보호비용에 비해 더 높다. 사회적 자원이 관점에서 볼 때, 치매 노인전문요양시설 보화가 치매환자에 대한 적절한 선택이 될 수 있다. 치매노인 전문요양보호는 장기보호, 특히 ADL 의존도가 높은 치매노인에서 보다 경제적이다. 셋째, 치매의 정도가 심해질 수록 시설보호이 비용 효과성이 커진다. 본 분석에서 고도 이상에 해당하는 치매 노인에게는 치매가족의 비공식보호가 전문요양시설에 비해 비용이 월등하게 높다는 사실을 확인할 수 있었다. 이러한 사실은 치매 정도가 심할수록 공적인 노인 전문요양시설 보호의 정당성을 더욱 강력하게 확인시켜준다.

영문 초록

The purpose of this study is to estimate economic burden of caring for demented elderly by caregivers, and then to discuss policy implications for the estimated outcomes. For this research, a set of caregivers living with demented elderly were surveyed. Among collected data, date for 81 caregivers were used for this study. Cost measures include the following three items: caregiver's direct costs, caregiver's indirect, caregiver's psychic costs. Findings and implications of this study are summarized as follows. First, the larger component of this cost is indirect costs. The total monthly cost of providing informal care to the demented elderly was estimated to be 806.000 won per patient in 2002. The indirect cost is the most of all categorized costs. That is, indirect cost is 493,000 won, with the direct costs being 289,000 won and psychic costs being 24.000 won. In this context, indirect costs are evaluated to be more significant item in social considerations for caregivers of the demented elderly. Second, for severely demented elderly, the cost encountered by nursing home facilities was found to be lower than the cost of informal care by families. From the social resources perspective, nursing home care may be an appropriate choice for dementia patients. Nursing home care is less expensive than family care for long-term care. Especially, for the demented elderly with severe or medium dependence. Third, compared to costs for informal family care, costs for caregiving by facilities was found to be lower relatively with disease severity. Because all aspects of costs increase with severity, and further indirect cost increase greatly by caregiving time's rapid expansion with disease severity.
