노인의 난청정도와 단어인지도에 따른 의사소통 어려움

국문 초록

본 연구는 노인의 난청정도와 단어인지도에 따른 의사소통 어려움에 대한 정량적 및 정성적 분석을 통해 임상세팅에서 노인의 의사소통 어려움을 예측하고 상담 등의 재활에 효과적으로 적용할 수 있는 지표 마련을 위한 기초 자료를 제공하는데 목적이 있다. 감각신경성 난청이 있는 60~80세 노인 67명을 대상으로 순음청력검사, 단어인지도 검사 및 의사소통 어려움에 대한 설문지 검사를 실시하였다. 노인의 난청정도에 따른 단어인지도를 살펴본 결과, 난청 정도가 심해질 수록 단어인지도가 유의하게 감소하였으며 중도 난청 이하의 그룹과 중고도 난청 이상의 그룹 간 유의한 차이를 나타내었다. 난청정도와 의사소통 어려움의 정도 간 유의한 상관을 보였으며, 경도 난청은 듣기에 때때로 어려움을, 중도 난청은 때때로 또는 거의 어려움을 보이며, 중고도와 고도 난청의 경우 듣기에 거의 어려움을 보이거나 항상 어려움을 갖는 것으로 나타났다. 단어인지도와 의사소통 어려움에서도 유의한 상관관계를 보였다. 본 연구 결과, 노인의 의사소통의 어려움의 정도와 유형은 난청정도와 단어인지도를 통하여 부분적으로 추정할 수 있으나 특정 범위의 난청정도와 단어인지도에서는 노인 개개인의 어려움의 정도가 다양하였다.

영문 초록

The purpose of this study was to examine the communication difficulties through quantitative and qualitative analysis in regards to the degree of hearing loss and word recognition in elderly with hearing loss in order to provide information for rehabilitation. 67 elderly participants with mild to severe sensorineural hearing loss were tested in pure-tone audiometry, word recognition, and questionnaire regarding communication difficulties. As the degree of hearing loss increased, word recognition significantly decreased, and there showed to be a significant difference for word recognition scores between below moderate hearing loss group and above moderate to severe hearing loss group. Communication difficulties were revealed "sometimes" for mild hearing loss, "sometimes" or "frequent" for moderate hearing loss, and "frequently" or "always" for moderate to severe hearing loss. Word recognition and communication difficulty showed significant correlation. The results indicate that certain degree of hearing sensitivity and word recognition scores can predict the degree and types of communication difficulties in elderly. However, it was also found that individual degree of difficulties varied in specific range of hearing sensitivity and word recognition scores.
