근거기반실무(Evidence-based practice)에 대한 언어재활사의 인식 및 실태 조사

국문 초록

근거기반실무(evidence-based practice, 이하 EBP)는 가변적이고, 불확실하며, 개연적인 특성을 가진 의사소통장애인의 중재에서 최선의 방안을 선택하는 전문가의 의사결정을 돕는다. 서비스의 질 향상을 통해 향후 언어재활이 보다 경쟁력 있는 직무로 거듭하기 위해서는 현시점에서 근거기반실무에 대한 실태 파악이 요구된다. 이에 본 연구의 목적은 다음과 같다. 첫째, EBP에 대한 국내 언어재활사의 수행능력 및 인식을 조사하고 둘째, 언어재활사의 특성에 따라 EBP에 대한 언어재활 수행능력과 인식에 차이가 있는지 확인하고자 한다. 설문은 서울, 경기, 강원, 경상지역에서 근무하고 있는 언어재활사 63명을 대상으로 시행되었다. 반응양식은 자기보고 형식에서 널리 인용되는 Likert 5점 척도를 수정하여 사용하였다. 응답결과를 살펴보면, EBP는 언어재활사의 의무이며(4.05점), 과학적 근거를 제시하고(4.11점), 이를 적용한 치료가 필요하며(4.11점), 관련 교육이 있다면 참가할 의향이 있다(4.29점)고 답하여 그 필요성을 긍정적으로 인식하고 있음이 확인되었다. 그러나 실제적으로 임상현장에서의 EBP를 적용하고 있는가를 묻는 항목(예: 개별치료 계획수립 시 EBP 연구자료 참고, 해외논문 이해 및 비평 등)에서는 상대적으로 낮은 3점대로 응답되었다. EBP 수행 실태에서 대학/종합 병원에 일하는 언어재활사가 타 기관의 언어재활사보다 상대적으로 높은 적용을 보고하였는데 이는 EBP가 학제 간 연구를 근간으로 한다는 사실을 뒷받침하였다. 본 연구 결과를 종합하여 향후 개설될 EBP 교육 방향은 언어재활사의 인식 개선보다는 기술의 실제적 활용에 초점을 맞춘 방향으로 계획되는 것이 바람직함을 제안하였다.

영문 초록

Evidence-based practice(EBP) helps to guide best decision making in clinical intervention for individual communication disorders. In order to improve clinical services, our study attempted to survey the perception and skills/knowledge on EBP. We also investigated differences of the perception and skills/knowledge according to characteristics of respondents. A total of 63 speech-language pathologists(SLPs) responded to a questionnaire examining EBP. SLPs scaled to each item of the survey using a five-point Likert scale of 1 to 5(1 being not at all and 5 being extremely). Results are as follows. EBP would be very important for the SLP's profession(4.05), EBP would provide scientific evidence(4.11), SLP should apply research evidence to the clinical intervention(4.11), and they were willing to participate in EBP workshop(4.29). However, low scores were shown in the items on applying EBP to their own clinical settings. Furthermore, skills/knowledge of SLPs who are working in the tertiary hospitals were higher than those in other settings such as private center, social welfare center. Based on the results of the survey, the majority of Korean SLPs considered that EBP was important in improving the quality of speech-language therapy services. Meanwhile, SLPs took a passive attitude to use skills/knowledge on EBP. This finding supported the fact that EBP is based on interdisciplinary studies integrating individual clinical expertise with the best available external clinical evidence from systematic research. EBP education based on the practical skills/knowledge would make SLPs have more professional position and competitive power in health and rehabilitation fields.
