국문 초록

혈관 확장성 운동 실조증(ataxia-telangiectasia)은 피부 및 결막의 모세혈관 확장증, 진행성 소뇌성 실조증 및 면역결핍으로 인한 반복되는 감염, 방사선 감수성, 악성질환의 소인을 특징으로 하는 드문 상염색체 열성 유전질환이다. 저자들은 걷기 시작하면서 실조성 보행을 보였고, 뇌 자기공명영상에서 소뇌 위축을 보였으며 검사 결과에서 혈관 확장성 운동 실조증으로 진단되었으며, 만 11세부터 사망 전 1년 동안에 점진적인 gamma-GTP의 상승을 보인 혈관 확장성 운동 실조증 1례를 경험하였기에 문헌 고찰과 함께 이를 보고하는 바이다.

영문 초록

Ataxia-telangiectasia is an autosomal recessive disorders characterized by cerebellar ataxis, oculocutaneous telangiectasia and frequent respiratory infections due to immunoincompetence. Ataxia usually appear by age of 2 years with most patients need wheelchairs for morbility by early teenage. Speech and eye movements are also affected. Other important features are immunodeficiency, a high level of serum α-fetoprotein concentration, growth retardation, telangiectasia and a very high risk of a lymphoid tumor. Patients also show an increased sensitivity to ionizing radiation. We report a case of a 7-year-old girl who had ataxic gate, conjunctival telangiectasia, and frequent upper respiratory infection. Her alpha-fetoprotein was elevated and the serum IgA was decreased. The brain MRI showed prominent cerebellar atrophy. From the 1 st year of life to death, the level of serum gamma-GTP became steadily elevated up to 10 times of a normal level.
