ADHD 아동대상 부모훈련 연구 현황과 개선방안: 2006년～2015년 국내연구 중심으로

국문 초록

임상에서 진단되고 투약치료 중인 ADHD 아동의 부모에게 실시된 부모훈련 연구의 현황을 알아보기 위해 2006년부터 2015년까지 10년간 국내 석․박사 학위논문과 학술지에 게재된 논문 중 근거기반치료로 인정된 Barkley의 행동적 부모훈련 프로그램을 기초로 시행되었고 메타분석이 가능한 연구결과를 보고한 4편의 연구를 선정할 수 있었다. 부모훈련 프로그램의 주 내용은 ADHD 개관, 부모의 분노 스트레스 다루기, 순종/독립적인 놀이증가, 효과적인 명령법, Think Aloud, 공공장소 내 행동조절 방법, 의사소통 방법, 부모자신에 대한 인식, 자녀수용, 행동수정(토큰 경제, 긍정적 관심주기, 반응대가, Time-out) 등 이었다. 독립변인은 ADHD 아동대상 부모훈련 프로그램이었으며 종속변인은 각 연구의 성과 측정치였다. CMA2를 활용한 메타분석 결과, 첫째, ADHD 아동대상 부모훈련 프로그램 전체 평균 효과크기는 .685였다. 둘째, 종속변인 별로 평균 효과크기는 부모양육태도 및 관계는 .865, 아동의 변화는 .691, 부모내적변화는 .544였다. 셋째, 프로그램 유형에 따른 평균 효과크기는 Barkley의 행동적 부모훈련 프로그램은 .664, Barkley의 부모훈련 프로그램을 포함한 수정 프로그램은 .718였다. 임상군을 대상으로 한 매우 열악한 국내 연구의 현황을 논의하였고, 연구의 제한점과 추후 고려해야할 사항을 논의하였다.

영문 초록

The purpose of this study was to examine the evidence and recommend suggestions for Barkley s behavioral parent training program for parents of children with clinically diagnosed, medically treated ADHD in Korea. Journals and doctoral theses on this program published between 2006 and 2015 in Korea were searched. Four studies met inclusion criteria. Program contents included: ADHD overview; handling parental anger/stress; increasing obedience/independent play; effective command; thinking aloud; controlling behavior in public places; how to communicate; perception of parents; adoption of children; and behavioral modification techniques. The independent variable of this study was parent training program for ADHD children, and dependent variable was each experimental treatment factors. Meta-analyses with CMA2 were conducted. Effects sizes were moderate to high, and as follows: average total effect size .685; parent rearing attitude .865; changes in children .691; parents’ internal changes .544. The average effect size for Barkley’s training program was .664, while the training program plus medication was .718. The lack of sufficient research on the use of this program in Korea, limitations, and future research considerations are discussed.
